CONTRACT The undersigned property owner(s), hereinafter called the “Owner,” does hereby contract and authorize the undersigned South B end Roofing & Siding Company, Inc., hereinafter called the “Contractor,” to furnish the necessary materials and labor to install the improvements listed below, subject to the following terms and conditions, on the premises located at: ADDRESS IMPROVEMENT SPECIFICATIONS* Terms: Contract Price Sales Tax Down Payment with balance due upon completion. Acceptance of this contract must be made on or before: 45 days Approximate Start Date: Will Call to Schedule *If the reasonably detailed description of the proposed improvement(s) in this contract does not include the specifications for the improvement, the Specifications, will be provided to the Owner before commencing any work and this Contract is subject to the Owner’s separate written and dated approval of the specifications. This contractor carries insurance for public liability, property damage and workmen’s compensation. The Contractor agrees to remove all waste materials from the premises upon completion of the work. All surplus material remains the property of the Contractor. All material warranties are those specified by the manufacturer furnishing the materials. If the customer does not have a satisfactory credit rating, this contract may be cancelled by the Contractor. This Contract shall not be binding on the Contractor unless and until it is approved by an officer in preparation of work and material specified under the above improvement specifications. All aluminum storm door, window, and special order products cannot be cancelled, and the full contract price shall be paid the contractor as liquidated damages for the breaking of the contract. If all work has been completed and payment is not made, then the contractor will have the rights to hire an attorney and file a lien if needed. All legal costs involved will be added to the contract and a 1.5% interest per month to any unpaid balance. The undersigned Owner hereby acknowledges that he/she has read the contract and understands that all materials to be furnished and work to be done are shown under the Improvement Specifications listed above. It is further acknowledged by the undersigned that this contract constitutes the entire agreement between the parties and that no other agreements or stipulations verbal or written, have been made or implied. Date: _______________________ Scope of work includes the home and the detached garage. 1. Remove existing aluminum siding and trim and haul to recycling center. 2. Prepare walls for siding installation. 3. Apply 3/8” foam siding underlayment to the entire area. 4. Remove and re-install downspouts as needed. 5. Install Mastic Rigid Vinyl Siding in accordance with the manufacturer’s specifications. (See product options below) 6. Use appropriate accessories where required. 7. Build-out window frames and wrap all with aluminum. 8. Replace existing shutters with new, vinyl shutters in existing locations. Additional shutters may be added for additional cost. 9. Build-out fascia as needed and wrap with aluminum. 10. Remove and re-install light fixtures (new light fixtures can be installed if provided and assembled from the homeowner). 11. Clean up worksite and haul away all work-related debris. All recyclable materials will be recycled. 12. Additional work needed to complete these specifications, but not included in these specifications will be performed on a time and material cost basis as an addition. 13. The manufacturer will provide the material warranty. 14. South Bend Roofing will provide a 5-year service warranty.
