<br /> <br /> <br />INTER-OFFICE MEMORANDUM <br /> <br /> DEPARTMENT OF PUBLIC WORKS <br /> DIVISION OF ENGINEERING <br /> <br /> <br />TO: Theresa Heffner, Clerk <br />Board of Public Works <br />FROM: Sarah Barber, Director of Engineering Services <br />SUBJECT: Change-Order #2 Point Repair & Maintenance Allowance <br />122-019 Sewer & Manhole Rehabilitation <br />DATE: June 10, 2025 <br /> <br />The Division of Engineering requests the Board of Public Works approval of Change Order No. 2 <br />for Project No. 122-019 Sewer & Manhole Rehabilitation. Change Order No. 2 addresses the <br />repair of a collapse discovered at the Lindsey Avenue sewer rehabilitation location between N. <br />Scott Street and Cushing Street. The collapse was discovered during cleaning of the conduit by <br />the project contractor and was determined to be damaged beyond the extent of being able to <br />Curried-In-Place-Pipe (CIPP) line this segment. The contractor has given an estimate instead of <br />an exact cost due to the inability to see the entire pipe beyond the collapse point. The extent of <br />the repair will be determined after excavation and the collapse can better be visually assessed. <br /> <br />The change order amount requested to be approved by BPW is for the allowance for a worst- <br />case scenario cost of needing to replace the entirety of the line segment and will be less if only a <br />partial replacement is needed. This change-order is a time and materials contract format so <br />unspent allowance by the end of the repair and project will return to wastewater capital funding to <br />support other projects. No additional time is needed to be added to the Sewer & Manhole <br />Rehabilitation project for this work. <br /> <br />Original Contract Amount: $1,529,590.50 <br />Previous Change Order No. 1: $40,832.75 <br />Current Change Order No. 2: $304,750.00 <br />Percentage Increase by Change Order No. 2: 19.41% <br />Total change for entire project: 22.59% <br />New Project Total: $1,875,173.25 <br />Additional Time: 0 days <br />New Completion Date: 3/17/2026; same as original contract <br /> <br />For questions regarding this BPW change order request, please contact Sarah Barber <br />(sbarber@southbendin.gov; 574-235-9805) at the Division of Engineering. <br /> <br />
