We have also <br />enclosed the following documents for your review and execution, which if indicated, may <br />also need to be executed in the presence of a notary: <br />1. City of South Bend Accounts Payable Voucher; <br />2. Taxpayer Identification Number Request; and <br />3. Permanent Easement (Must be notarized). <br />If more convenient to you, please feel free to give me a call to schedule an appointment <br />where we can review the documents with both of you and have one of our staff members <br />notarize them for you. Otherwise, please send one completed copy of this letter and the <br />other enclosed documents to the address indicated above.
