The goals of the <br /> County,City, and the Housing <br /> Authority are complementary. <br /> FY 2025-2029 Five-Year City of Mishawaka They are incorporated in the <br /> Consolidated Plan Five-Year Consolidated Plan and <br /> the Annual Action Plans. <br /> 2023 Childhood Lead Indiana Department of Health Blood Lead Levels are included <br /> Surveillance Report Lead&Health Homes Division in the Needs Assessment. <br /> Table 3—Other local/regional/federal planning efforts <br /> Describe cooperation and coordination with other public entities, including the State and <br /> any adjacent units of general local government, in the implementation of the Consolidated <br /> Plan (91.215(1)) <br /> The City of South Bend is the Participating Jurisdiction(PJ)for the St.Joseph County Housing Consortium <br /> and is the administering agency for the HOME funds.The City of South Bend's Department of Community <br /> Investment's Neighborhoods Division is also the administrating agency for the ESG funds. Close <br /> coordination is maintained with other city departments such as the Fire Department, Public Utilities, and <br /> Parks & Recreation, as well as County departments such as the Health Department, Transpo, the Area <br /> Planning Commission, and the Indiana Balance of State Continuum of Care. Coordination with various <br /> non-profit organizations, such as the Center for the Homeless,Youth Services Bureau,and the Food Pantry <br /> of Northern Indiana helped aid in the planning process and develop priorities. The City works closely with <br /> regional organizations and County staff to address projects and activities that extend beyond the City limits. <br /> The City and the County agencies have a good working relationship. <br /> The Indiana Balance of State CoC coordinates Con Planning, CoC strategic planning and ESG <br /> policies/priorities/allocations.
