PLAN AND EQUIPMENT QUESTIONNAIRE <br />The Signatory of this questionnaire guarantees the truth and accuracy of all statements and of all <br />answers to interrogatories hereinafter made. <br />1. In what manner have you inspected this proposed work? Explain in detail. <br />Larson- Danielson estimating staff has thoroughly reviewed and assessed the drawings <br />and specifications provided for bid purposes on this project and have communicated <br />with subcontractors and suppliers in evaluating the full scope of work for this project. <br />2. Explain your plan or layout for performing the proposed work. <br />Larson -Danielson anticipates commencing project coordination upon receipt of a Contract, <br />Notice to Proceed, or Letter of Intent. We will then begin awarding subcontracts based on <br />sequence of work to be performed in order to begin on -site construction as soon as <br />possible. Work will be performed in common sequence of tasks as it relates to this project <br />that provides for the best quality end product. <br />3. The work, if awarded to you, will have the personal supervision of whom? <br />A Project Manager and Superintendent will be assigned to this project following <br />notification of the bid results. <br />*4. Do you intend to do the hauling on the proposed work with your own forces? <br />If so, give amount and type of equipment to be used. <br />If hauling is needed, this is to be determined following notification of bid results unless <br />stated otherwise in sunnlemental bid documents. <br />*5.
