ow'AFO",ONTZ- WAGNER <br />POWER • PERFORMANCE • PROGRESS <br />May 29, 2024 <br />Part II Attachments <br />(i) Please see attached copy of our certificate from the Secretary of State's Office <br />(ii) Koontz -Wagner Electric, Inc, Koontz -Wagner Construction Services, KW Construction <br />Services <br />(iii) There have been no determinations of violations of federal, state, or local laws. <br />(iv) Koontz Wagner is signatory to the labor agreement with IBEW Local Union 153. We <br />currently employ approximately 200 tradesmen. This project will be staffed by our in-house <br />field forces. <br />(v) All of our field staff are classified by the local union as either foremen, journeymen or <br />apprentices and they are all members of IBEW Local Union 153. <br />(vi) The electrical scope of work being performed by our field forces will be performed by <br />members of IBEW Local 153. By being signatory to this union, we are also participating <br />members of their apprenticeship program. Certificates of compliance are available from our <br />respective apprenticeship training organization (SBVJATC). We have included a letter from <br />our JATC stating our participation in their program. <br />(vii) All field members of our crews participate in the substance abuse program sponsored jointly <br />by IBEW Local Union 153 and the electrical contractors of the Northern Indiana Electrical <br />Contractors Association. These programs are in compliance with requirements as spelled out <br />in the IC regulations. <br />(viii) The surety that we will use is listed on the Bureau of Fiscal Service "Department of Treasury's <br />Listing of Approved Sureties". <br />(ix) We certify that we have no federal, state or local tax liens or tax delinquencies owed to any <br />federal, state or local taxing body for the preceding three years. <br />(x) Please see the list below of projects completed: <br />a.
