Summary description. This document should not exceed three pages and must include the following
<br />information, in the order in which it is listed for the nomination to be considered:
<br />1. Describe: a) the project, program, activity, or individual being nominated; b) how this nominee
<br />meets the award criteria; and c) how it resulted in significant outcomes. Be specific and give
<br />examples, facts, and figures to support your assertions —for instance, visitation figures, retail
<br />data, tax figures or percentages, demonstrated growth, demonstrated revitalization, specific
<br />preservation or conservation achievements, evidence of increased civic pride, etc. If the nominee is
<br />an individual, describe their accomplishments and contributions using the general criteria.
<br />2. Explain the significance of the cultural or natural heritage asset involved, and note if it is listed in
<br />any national, tribal, state, or local registry or inventory.
<br />3. Describe the role of the nominated organization, business, government entity, or individual and
<br />how it contributed to the success of the project, program, or activity. If partners, describe their
<br />roles and contributions.
<br />4. Provide final cost figures for the nominated project, program, or activity. Specify each funding
<br />source and the amount of money or proportion of costs /resources provided by each source.
<br />Ongoing or cyclical programs may provide startup and annual costs to date.
<br />5. If the nominee has previously won an award or been publicly recognized, please note and explain.
<br />6. Note the completion date of the nominated activity. If cyclical or ongoing, please elaborate.
<br />Images. Nine sets of 8 -12 color images (4" x 6" or larger), with captions and photo credits, are also
<br />required. If your photos are printed from digital images, please include a CD containing the images at a
<br />resolution of 400 dpi or greater, as well as a text file with captions and credit information. The images
<br />should show, as appropriate, before and after results, types of resources affected, people carrying out the
<br />project, and people using, enjoying, and learning from the historic cultural or natural assets.
<br />Optional documentation.
<br />Nominations may also include up to five clippings, brochures, reports, or other publications. They
<br />may also include no more than three letters of endorsement by preservation and civic organizations
<br />and professionals that demonstrate community support, significant outcomes, and high quality of
<br />preservation efforts. If you provide optional documentation, please provide nine copies of each item.
<br />NOMINATION FILING
<br />Include one signed original and eight copies of the completed nomination form and summary
<br />description, plus the required images, in a single, one and one -half inch, three -ring binder. Optional
<br />documentation may be provided in a separate pocket -style or accordion -style folder. All nomination
<br />material must be sent in a single package that is postmarked no later than November 1, 2006.
<br />Nominations will not be returned. All nomination materials, including photographs, become property of
<br />the ACHP and the PreserneAmerica Presidential Award program, and may be reproduced by the ACHP
<br />and its partners without additional explicit permission.
