hereby give my consent to authorize my employer known <br />as T. George Podell & Co., Inc. and the testing laboratory designated to conduct analytical and random <br />tests deemed necessary, on an ongoing basis, to determine the absence or the presence of Alcohol, <br />Amphetamines, Cocaine, Opiates (2000 ng/ml), PCP & THC in my body through the use of urine, hair, <br />blood, breath or any sample as specified by statute and regulation. <br />I give my consent to release the results of the test(s) and other medical information from the laboratory to <br />T. George Podell & Co., Inc. pursuant to statute or regulation with the condition that the results may not <br />be used in any criminal proceeding. If employee fails drug test, that employee will be subjected to the <br />items listed below. <br />After a first positive test, an employee must be: <br />• suspended from work for ninety (30) days; <br />• directed to a program of treatment or rehabilitation; and <br />• subject to unannounced drug testing for one (1) year, beginning the day the employee returns to <br />work. <br />After a second positive test, an employee must be: <br />• suspended from work for ninety (90) days; <br />• directed to a program of treatment or rehabilitation; and <br />• subject to unannounced drug testing for one (1) year, beginning the day the employee returns to <br />work, <br />After a third or subsequent positive test, an employee must be: <br />• suspended from work for one (1) year; <br />• directed to a program of treatment or rehabilitation; and <br />• subject to unannounced drug testing for one (1) year, beginning the day the employee returns to <br />work. <br />
