Groundwater & Environmental Services, Inc. <br />(GES) has been contracted by Honeywell International Inc. (Honeywell) to provide operation, <br />maintenance and monitoring services for the groundwater extraction systems that operate to <br />control groundwater at the Honeywell Plant Complex (Site) in South Bend, Indiana. <br />After 20 years of operation existing remediation well EW-3 has failed and needs to be <br />replaced. The new replacement remediation well (EW-3r) will be hooked up to <br />existing infrastructure to allow for extracted groundwater to be discharged under Site <br />Remediation Permit No. 004 issued by Department of Public Works, City of South Bend to <br />Honeywell on April 14, 2015. GES has prepared the following required forms for a new <br />remediation well adjacent to the Site in road easement between sidewalk and Bertrand Street: <br />BPW Encroachment Application; <br />• Long -Term Occupancy Request Application; and <br />• Excavation Permit Request Form. <br />
