SUPPLEMENTAL AGREEMENT NO. 2 <br />This Supplemental Agreement No. 2 is made and entered into thisOo day of CeC. , 20E—� <br />by and between the City of South Bend, Indiana, acting by and through the Board of Public Works, <br />hereinafter referred to as the "LPA", and DLZ Indiana, LLC, 2211 East Jefferson Blvd., South Bend, IN 46615, <br />hereinafter referred to as the "CONSULTANT". <br />WITNESSETH <br />WHEREAS, the LPA and CONSULTANT did, on February 10, 2015 enter into an agreement for the <br />preparation of construction documents for the construction of the Road Reconstruction on Bendix Drive, <br />Designation Number 1400634. <br />WHEREAS, during the course of design, certain additional services not included in the original <br />contract dated February 10, 2015 and Supplemental Agreement Number 1 dated April 11, 2017, have <br />become necessary; specifically: <br />Subsurface Utility Engineering to verify the elevation of known underground Utilities <br />along the project corridor. <br />NOW THEREFORE, the LPA and the CONSULTANT desire to amend the above referenced Design <br />Contract as follows: <br />Delete Subsection II.C.3J. of A endix A in its ntiretw+,: <br />A. Perform Utility Designating, ASCE Quality Level A. <br />a. The CONSULTANT shall perform up to 25 test holes to locate the elevation <br />of either existing water mains, gas mains and/or fiber optic line along <br />Bendix Drive from north of Lathrop Street to the toll road bridge <br />(approximately 4,900 LFT). <br />b. From the beginning of the project to Boland Drive, the CONSULTANT shall <br />perform a test hole every 200 LFT following the existing water main (14 <br />test holes). <br />C. The CONSULTANT also anticipates an additional 6 text holes near the <br />beginning of the beginning of the project grouped between station 65+50 <br />and 69+00 and 5 more holes at the north end of the project for an existing <br />water main and fiber optic line. <br />d. All holes will be backfilled with native soils and compacted in 6" lifts. <br />e. The CONSULTANT shall patch all holes with bag mix concrete. <br />Perform Utility Designating, ASCE Quality Level B. <br />Utility Designating, ASCE Quality Level B, will utilize surface geophysical <br />methods to interpret the possible presence of underground utilities, both <br />public and private, and then employ electromagnetic equipment to detect <br />and designate the selected utilities.
