Beginning and end times <br />iii. Whether you will be using or closing a residential street or sidewalk <br />2. A Certificate of Insurance naming the City of South Bend as an additional insured must be <br />attached to your application <br />3. Applications MUST be received at least FOUR (4) WEEKS in advance <br />4. Signature and contact information is required <br />5. Please make the business/agency/property of where you will be meeting to start your event is <br />aware of your event. <br />6.
