Section II Plan and Equipment Questionnaire <br />1. Plan for proposed work. Permit process can begin when the notice of award is issued. Contracts will be <br />sent to subcontractors. Abatement can begin 2 weeks after NESHAP is submitted.: <br />a) File NESHAP, Apply for permits, Schedule utility disconnects <br />b) Set up traffic control <br />c) Begin abatement <br />d) Perform handwork for separation/salvage <br />e) Demolish building with hydraulic excavators <br />f) Load out debris <br />g) Replace sidewalks/curbs <br />h) Backfill <br />2. Subcontractors on past Public Works projects <br />a) Water Well Solutions, 825 E. North St. Elburn, IL 60119; Sealed well on project <br />b) J Ave Development, 16430 New Ave Lemont, IL 60439; Trucking <br />c) Earth, Inc. 810 N. Arlington Heights Rd. Itasca, IL 60143; Trucking of stone <br />3. Subcontractors still to be determined. It is anticipated that the abatement, trucking, traffic control, and <br />concrete work will be subcontracted. Information will be provided to the City as these subcontractors <br />are selected. The City will also be notified of any additional subcontractors that will perform work on <br />the project. <br />4. Cat 349, Cat 336, Cat 321, Cat 314 excavators, Cat 950 loader, Cat 262 skidsteer. Rental man lifts will <br />also be used on the project. <br />5.
