City of South Bend <br />EAP Addendum 1 <br />Page 1 of 2 <br />September 13, 2018, amended October 15, 2018 <br />DEFINITION OF ELIGIBILITY <br />For The New Avenues Employee Assistance Program <br />City of South Bend (hereinafter, referred to as "Company") shall include employees of City <br />of South Bend. The Company retains the authority to define the parameters of eligibility <br />for utilization of the EAP. <br />Please note that it is the Company's responsibility to notify New Avenues of any changes <br />of these assumptions of eligibility and to submit a monthly employee eligibility list (in our <br />suggested electronic format) no later than the 161" of each month. This is necessary so <br />that New Avenues can administer the EAP service according to the Company's needs. <br />Basic assumptions of eligibility are suggested but not limited to the following: <br />Definition of Eligibility <br />All eligible employees and their immediate family members living in the household of the <br />employee may initiate and/or participate in the Employee Assistance Program (EAP) <br />counseling process. Immediate Family is defined as one's spouse, domestic partner, <br />parents, grandparents, children (natural), adopted children, step -children, grandchildren, <br />siblings, and immediate in-laws (mother-in-law, father-in-law, brother-in-law and sister -in - <br />la w). <br />The Company has determined specific employee groups who are eligible for the EAP to <br />include all active full-time and benefitted part-time employees; includes immediate family <br />members including same -sex domestic partners. Regular part-time, seasonal, temporary, <br />interns or contractual employees are not eligible for the EAP. The Company will submit to <br />New Avenues on a monthly basis the list of covered employees. It is recognized that the <br />EAP is extended to more employees than those who elect the insurance program. <br />College Students <br />A college student, living outside of the home, shall be eligible for receiving EAP services, <br />if the EAP has a provider in the college's community. <br />Children of Employees who are Not Residing in the Home of the Employee <br />Dependent children who are not residing in the home`of the employee are eligible up to <br />age 26 if they are on the employee's health insurance. <br />Other Individuals Living in the Home of the Employee who are Not Part of the <br />Immediate Family . <br />Other persons living in the home of the employee may be seen if the employee is actively <br />participating in the counseling process. <br />
