Boyles, P.E. <br />Professional Engineer No. 10809510 <br />CONTRACTOR: HRP Constructtion <br />Change Order No, 1--10/23/18 <br />PROJECT: Mishawaka Avenue — Roto-Rooter Jetter Removal (Division 1) <br />Project No. 118-069R <br />SUBJECT: Change Order Explanation <br />ISSUE Date: October 23, 2018 <br />This Change Order to the Contract document shall supplement, amend, and shall become <br />a part of, and be incorporated as an Addendum to the Contract. <br />The purpose of this Change Order is to add costs associated with conducting a more <br />detailed video inspection and decrease the overall costs by adjusting the removal means. <br />The video inspection further investigated the lateral connections to get a better picture of <br />the Roto-Rooter hose issues. This method determined that jetter head removal is not <br />feasible without risking extensive damage to the private lateral. The hose will now be <br />removed from the main by mechanical means in lieu of full excavation. <br />What we do today makes a difference! <br />
