Martin, Clerk <br />Board of Public Works <br />FROM: Sue Ellen Doudrick <br />SUBJECT: Division 2 Change Order for Project No. 118-069 exceeding 20% <br />Project Name: Mishawaka Avenue — Rota Rooter Jetter Removal <br />DATE: September 5, 2018 <br />This project had two Divisions; Division 1: Mishawaka Avenue and Division 2: North Shore Drive. <br />Unfortunately, the project title is a bit misleading for the entire scope of work. This memo was <br />prepared for Division 2: North Shore Drive only. <br />Division 2 work is completed. The scope of work included: removal and replacement of eight (8) <br />catch basins and inlets on North Shore Drive (between Michigan and Niles Avenue). During <br />construction, unforeseen issues arose. The Contractor, Selge Construction, performed necessary <br />additional work on the North Shore Drive (Division 2) Project ##118-069R which added $19,507.00 <br />(36.95%) to the original project ($52,800.00). Total cost is now $72,307.00. <br />Description of work: <br />When the Contractor opened up two (2) of the eight (8) catch basin (CB) structures and began <br />the removal process, damaged or blocked storm leads were discovered. The Contractor notified <br />the Construction Inspector and an investigation by the City's Sewer Department was required at <br />one location to pinpoint the issue. A description of the two locations where additional work was <br />required are as follows: <br />9. SE Corner of Michigan & North Shore Drive. Installing new storm sewer pipe from the CB <br />to storm manhole located between Leeper Avenue and Michigan on North Shore Drive. <br />The existing lead pipe had a sandy debris blockage several feet into the lead pipe. This <br />blockage created backups within the CB which lead to excessive puddling at the <br />intersection's southeast corner during rain events. This new pipe installation will alleviate <br />the flooding issue and was an opportunity for creating a storm separation from the <br />combined sewer along North Shore. Alternative relocation of lead presented major utility <br />conflicts or trenching across Michigan and North Shore Drive. <br />2. NW Corner of Niles Avenue & North Shore Drive. Installing new storm sewer pipe from <br />the CB to the nearest manhole in Niles and North Shore. The existing pipe was clogged <br />with concrete debris and soils, and one of the joints was offset. These issues create <br />puddling at the northwest corner and sent excess water to CBS farther downhill. This new <br />pipe installation will alleviate flooding and provide additional capacity and hydraulic <br />efficiencies. Notice pictures below of debris found within CB and within lead pipe. <br />
