required more frequently by the discharge permit, a periodic <br />compliance .report on forms provided or prescribed by the Director <br />indicating the nature 'and concentration of prohibited or regulated <br />substances in the effluent. Included in the report shall be a record <br />of all measured or estimated average and maximum daily flows <br />during the re ortin eriod computed by verifiable techniques as <br />required by the a `licable discharge permit.. In cases where the <br />Pretreatment Standard requires coml2liance. with a Best <br />Management _Practice (BMP) or pollution prevention alternative, <br />the User must submit documentation required by the Director or <br />the Pretreatment Standard necessary to determine the compliance <br />status of the User. <br />b. Users ._..,. shall _ be required to have a Certified Industrial <br />Wastewater Operator of the State of Indiana sin all periodic <br />compliance reports indicating whether pretreatment Standards and <br />Requirements are being met on a consistent basis, and if not <br />whether additional operation and maintenance and /or additional <br />pretreatment is required to meet the pretreatment standards and <br />requirements, unless otherwise indicated in the User's pemit. The <br />Director shall s ecif y the certification class required in the User's <br />permit. <br />C. All neriodic com liance reports must be signed and <br />certified in accordance with Subsection 5)(a), of this Section 17- <br />14 and signed by an Authorized Representative as defined in <br />Section 17 -5.1 Division 1 of this Article. <br />d. All wastewater samples must be re resentative of the User's <br />discharge. Wastewater monitoring and flow measurement facilities <br />shall be p gperly operated, kept clean, and maintained in good <br />working order at all times. The failure of a User to keep its <br />monitoring facility in good working order shall not be rounds for <br />the User to claim that sample results are unrepresentative of its <br />discharge. <br />e. If a User subject to the reporting requirement in this section <br />monitors aLiy regulated Pollutant. at the gppropriate sampling <br />location more frequently than_xSubsec by the Director, using the <br />.eau „. q_,_' � <br />procedures prescribed in l "Analytical <br />equirements_ " and in S <br />_ ubsection (p)(ZL Sample Collection of <br />this Section 17 -10, the results of this monitoring shall be included <br />in the report. <br />49 <br />
