Each individual wastewater discharge permit will indicate a specific date <br />upon which it will expire. <br />,0(2) An individual wastewater discharge permit shall include such <br />conditions as are deemed.reasonably necessary by Director to prevent <br />Pass Through or Interference, _ protect the quality of the water body <br />receiviniz the treatment plant's effluent,_protect_ worker health and safety, <br />facilitate sludge management and disposal, and protect against dama a to <br />the POTW. <br />a. Individual wastewater -discharge permits must contain: <br />Q A statement that indicates the discharge <br />permit issuance date, exp_i_ration date and effective date; <br />ii) A statement that the wastewater discharge permit is <br />nontransferable without prior notification to the City in <br />accordance with Subsection (n )(2) "Individual Wastewater <br />Discharge _ Permit Cessation,__ Revocation Transfer, <br />Reissuance" of this Section 17 -10 and provisions for <br />furnishing-the new owner or operator with a copy of the <br />existing wastewater discharge permit; <br />iii Effluent limits, including Best_ Management Practices, <br />based on applicable Pretreatment Standards-, <br />iv Self monitoring, sampling_ reporting, notification, and <br />record - keeping requirements. These requirements shall <br />include an identification of pollutantsLor Best Mana ement <br />Practices) to be monitored, location, sampling <br />frequency, and sample type based on Federal, State, and <br />Local law. <br />O A statement of applicable civil and criminal penalties <br />for violation of Pretreatment Standards and Requirements, <br />and any applicable compliance schedule. Such schedule <br />may not extend the time for beyond that <br />required by applicable Federal State or Local law. <br />vi Requirements to control Slug Discharizes. <br />b. Individual wastewater_ discharge permits mU contain, but <br />need not be limited to, the following conditions: <br />Limits on the __average and/or maximum rate of <br />discharge, time of discharge,_ and /or requirements for flow <br />
