PLAN AND EQUIPMENT QUESTIONNAIRE The Signatory of this questionnaire guarantees the truth and accuracy of all statements and of all answers to interrogatories hereinafter made. In what manner have you inspected this proposed work? Explain in detail. Larson- Danielson estimating staff have thoroughly reviewed and assessed the drawings andspecifications provided for bidding u osed on this project and have communicated with subcontractors and suppliers in evaluating the full scope of work for this project. 2. Explain your plan or layout for performing the proposed work. Larson -Danielson anticipates commencing project coordination upon receipt of a Contract Notice to Proceed or Letter of Intent. We will then begin awarding.subcontracts based on sequence of work to be performed in order to begin on -site construction as soon as possible. Work will be performed in common sequence of tasks as it relates to this project that provides for the best quality end product. 3. The work, if awarded to you, will have the personal supervision of whom? A Project Manager and Superintendent will be assigned to this project following notification of the bid results. *4. Do you intend to do the hauling on the proposed work with your own forces? If so, give amount and type of equipment to be used. If haulints is needed. this is to be determined following notification ofbid results unless stated otherwise in su lemental bid documents. *5. If you intend to sublet the hauling or perform it through an agent, state amount of sub- contract or agent's contract, and if known, the name and address of sub -contractor or agent, amount and type of his equipment and financial responsibility: If hauling is needed this is to be determined following notification of bid results unless stated otherwise in supplemental bid documents.
