Klosinski, P.E., Assistant City Engineer <br />SUBJECT: Project No. 116-050R WWTP Digester Gas Utilization Project <br />Change Order No. 2 <br />DATE: April 12, 2018 <br />The Division of Utilities is requesting the Board of Public Works approval of Change Order <br />No. 2 for Project No. 116-05OR Wastewater Treatment Plant Digester Gas Utilization. . <br />Change Order No. 2 addresses the following changes: <br />COP 5 — Liquidated Damages for Substantial Completion <br />NET DECREASE FOR CHANGE ORDER NO. 2 ITEMS <br />Original Contract Sum <br />Net change by previously authorized change orders <br />The contract sum prior to this change order <br />By this change order, the project amount decreases by <br />The new contract sum including this change order is <br />This Change Order represents a total change of <br />Total change for the entire project <br />Time Extensions resulting from Change Order No. 2: <br />Substantial Completion: <br />Final Completion: <br />($ 30,000.00) <br />($ 30,000.00) <br />$ <br />754,000.00 <br />$ <br />30,827.00 <br />$ <br />723,173.00 <br />($ <br />30,000.00) <br />$ <br />693,173.00 <br />-3.98% <br />-8.07% <br />0 days <br />0 days <br />It is noted that the settlement offer for liquidated damages was for the collection of liquidated <br />damages for the Contractor's failure to meet the Substantial Completion deadline. The <br />liquidated damages for the Contractor's failure to meet the Final Completion deadline are hereby <br />waived by the Board of Public Works acceptance of Change Order No. 2. <br />The funding source for Change Order No. 2 is directed as a credit to the Division of Utilities <br />Wastewater Capital Account (642-0630-415-43-66) and Other Contractual Services (642-0630- <br />793-63-35). For questions regarding the above recommendation or the subject project, contact <br />Al Greek (Ext. 4210) or Jacob M. Klosinski (Ext. 9496) at the Division of Utilities. <br />
