Prepared by: Jones Petrie Rafinski <br />ADDENDUM NO.3 <br />To the Contract Document and Project Specifications for the <br />CHARLES BLACK CENTER <br />SOUTH BEND,_INDIANA <br />PROJECT NO.2015-0200 116-073 <br />SOUTH BEND PARKS <br />Date: Sept. 5, 2017 <br />To: All holders of the contract documents for the above project. <br />The items in this addendum are to become a part of the original documents as if included therein. Only the noted items are affected <br />and the remainder of the original contract document is to remain in its entirety. <br />This Addendum modifies the Contract Documents as follows: <br />PART I — GENERAL CLARIFICATIONS <br />PART 2 — SPECIFICATIONS <br />PART 3 — CONSTRUCTION DRAWINGS/DETAILS <br />PART 4 --- PRODUCT APPROVALS <br />PART 1—GENERAL CLARIFICATIONS <br />1. Ceramic tile in Rest rooms 116 and t 18 shall be 12" x 12" for floors, walls, and base of sink walls. Bull nose 4" x 12" for <br />remaining; wall base. <br />2. Lockers shall be Salsbury Industries Double tier "S' style #27364 (7) sets of 6 (3 wide) in women's locker room and (7) sets <br />of 6 (3 wide) in men's locker room, Cut sheet attached <br />3. Eliminate base bid allowance item la Charles Black logo in exercise room flooring = $1,000.00 <br />4. Add base bid allowance for Temporary heat, concrete and mortar mix premiums for cold weather construction=$10,000.00 <br />5. Contractors are approved to tap into and use owners electric and gas service, however the charges will be paid by contractor <br />immediately upon receipt of bill from owner, any and all damage to gas or electrical service is the responsibility of the <br />contractor. <br />6. Owners items in Weight room and flex space will be cleared out prior to Notice to Proceed. <br />7. Interior painted CMU to receive New- Two coats block filler with epoxy paint top coat <br />Existing — two coats epoxy paint <br />Follow Section 099100 Painting PART 2 PRODUCTS 2.01 SCHEDULE OF PAINTING M <br />8. Remove demolition of existing lockers from contract. Owner will have lockers removed prior to Notice to Proceed. Removal <br />of concrete locker base still in contract. <br />9. If Alternate #9 -replace existing gym lights- is accepted, Contractor to remove and return to owner existing lights. Contractor <br />to include in price for alternate. <br />10. Climbing rope in gym shall be Porter 00115-16 or equal. <br />11. The final Soil management plan is attached and is to be included in contract bid as an itemized price. <br />12. The roofing/ insulation and roof structure over the existing restroom/storage area is to be removed down to load bearing <br />walls and replaced with new structure and roof/insulation per sheets D 102 and S 101. <br />13. Existing Restrooms in locker rooms may be used by contractors until removed. The two restrooms next to Community Room <br />may be used during construction but toiletries must be supplied by contractor and any damage or misuse of the restrooms is <br />the responsibility of the contractor. Also the rest rooms must be cleaned prior to issue of substantial completion. <br />14. All Fire Rated Glass shall be Pyran Platnum L or equal. <br />15.
