If the designation of an Authorized Representative is no longer accurate <br />because a different individual or position has responsibility for the overall <br />operation of the facility or overall responsibility for environmental <br />matters for the company, a new written authorization must be submitted <br />to the Director prior to or together with any reports to be signed by the <br />Authorized Representative. <br />B. Additional Sampling <br />If the permittee monitors their wastewater discharge for any pollutant more <br />frequently than required by this permit, using test procedures in 40 CFR 136 or <br />amendments thereto or otherwise approved by U.S EPA or as specified in the <br />permit, the results of such monitoring shall be included in any calculations of <br />actual daily maximum or monthly average pollutant discharge, and results shall <br />be reported in the specific time frame report submitted to the Control Authority. <br />C. Notification of Batch or Infrequent Discharge <br />1. Within 30 days of the effective date of this permit, the permittee shall commence <br />to notify the City of South Bend via e-mail twenty-four (24) hours in advance of <br />all batch or infrequent discharges to the City's sewer so that during this time <br />compliance monitoring may be conducted. The subject line shall include the <br />words "discharge notification" followed by the outfall number, anticipated time <br />of discharge, and the anticipated volume of discharge. The permittee may request <br />a modification of notification requirements. All requests for modification of <br />notification requirements are subject to approval by the City of South Bend. The <br />notification shall be made via e-mail to Pretreatment SoythBendlN. Gov. <br />D. Noncompliance Notification and Re -Sampling <br />The permittee shall notify the Control Authority immediately of discharges that <br />may cause problems to the POTW, including any slugload or accidental <br />discharge. <br />2. If sampling performed by the permittee indicates a violation, the permittee shall: <br />a. notify the Control Authority orally within twenty-four (24) hours of <br />becoming aware of the violation and; <br />b. submit a written follow up report within five (5) days of becoming aware <br />of the violation and; <br />r. <br />
