Klosinski, Assistant City Engineer <br />SUBJECT: Agenda Request Form — Project Completion Affidavit <br />114-075 Wastewater Treatment Plant Grit and Screenings Improvements <br />DATE: March 31, 2017 <br />The Division of Utilities is submitting the Project Completion Affidavit, Certificate of Substantial <br />Completion and Maintenance Bond for Project No. 114-075 Wastewater Treatment Plant Grit and <br />Screenings Improvements to the Board of Public Works for approval at the April 11, 2017 Board <br />Meeting. <br />Background <br />This project included the replacement of aging equipment at the Headworks Building. Grit washers, <br />cyclones, compactors, conveyor equipment, piping upgrades, and automated valves were included <br />in the upgrades. In addition, SCADA 1 HMI upgrades were provided to incorporate additional <br />automation and flexibility for the Operations and Monitoring of the upgraded equipment. <br />The funding source for the project is Utilities, Wastewater, Treatment and Disposal Equipment <br />Account (642-0630-415-43-66). <br />If there are any questions regarding the subject project, please contact Al Greek (Ext. 4210) or <br />Jacob M.
