QtSu <br />REGULAR MEETING <br />1 more or less surge <br />suppressor <br />1 more or less flatbed <br />scanner <br />MARCH 15, 1993 <br />NO BID <br />NO BID <br />NO BID <br />NO BID <br />Upon a motion made by Mr. Leszczynski, seconded by Mrs. Mueller <br />and carried, the above bids were referred to the City Controllers <br />office for review and recommendation. <br />OPENING OF BIDS - RENTAL OF NEW UNIFORMS FOR VARIOUS DEPARTMENTS <br />AND BUREAUS <br />It was noted that this was the date set for the receiving and <br />opening of sealed bids for the rental of new uniforms for various <br />departments and bureaus. The Clerk tendered proofs of publication <br />of Notice in the South Bend Tribune and the Tri-County News which <br />were found to be sufficient. The following bids were opened and <br />read: <br />MECHANICS LAUNDRY & SUPPLY, INC. <br />711 East Vermont Street, Suite 200 <br />Indianapolis, Indiana 46202 <br />Bid was signed by Mr. R.M. Peele, Vice -President Marketing, <br />Non -Collusion Affidavit was in order, Non -Discrimination <br />Commitment form was completed and a Bid Bond in the amount of <br />$5,000.00 was submitted. <br />00M <br />GROUP I (11 uniforms each - pants and shirts, 5 changes per <br />week) <br />Rental price per employee per week: SEE ALTERNATE <br />GROUP II (11 uniforms each - pants and shirts, 5 changes per <br />week) <br />Rental price per employee per week: SEE ALTERNATE <br />GROUP III (3 coveralls each - 1 piece, 1 change per week) <br />Rental price per employee per week: SEE ALTERNATE <br />GROUP IV (3 shop coats or smocks each - 1 piece, 1 change <br />per week) <br />Rental price per employee per week: SEE ALTERNATE <br />REPLACEMENT COSTS: <br />Uniform pants $ 9.75 <br />Uniform shirts $ 7.50 <br />Uniform coveralls $16.50 <br />Shop coats/smocks $ 8.20 <br />Dress uniform shirts <br />Dress uniform pants <br />Mechanics proposes the following alternate bid to the <br />specifications as outlined on the proposal sheet. <br />1. Contract shall be for five (5) years. <br />2. Pricing shall be fixed for years one (1) and two (2). <br />3. Pricing shall be increased a maximum of seven per cent <br />(70) in year three (3) and shall be fixed at that rate <br />for years three (3) and four (4). <br />NOTE: This price increase is by mutual consent and if <br />the City of South Bend does not allow the price <br />increase, Mechanics Laundry & Supply, Inc. shall have <br />the right to cancel the contract with thirty (30) days <br />notice or continue to service at the existing rates. <br />
