August 3, 2010 <br />Department of Community & <br />Economic Development <br />City of South Bend <br />227 West Jefferson Blvd., Suite 1200 <br />South Bend, IN 46601 <br />Attention: Mr. Bill Schalliol <br />RE: TOPOGRAPHIC SURVEY ZIOLKOWSKI CONSTRUCTION SITE/TREELAWN <br />SURVEYING, SOUTH BEND, INDIANA <br />Dear Bill: <br />Thank you for the opportunity to present our proposal for surveying for the above referenced project. <br />We have appreciated being able to be of service to you in the past and hope that we can assist you <br />with this project. <br />We have reviewed the documentation and project limits for this project and offer the following <br />proposal. <br />SCOPE OF SERVICES <br />Topographic survey shall include the following: <br />• Utility locates will be performed on Ziolkowski Construction site located at the Southeast <br />corner of Lafayette Blvd. and Garst Street. <br />• Utilities and topographic features between the existing edge of pavement and the right of <br />way lines for the following street: Lafayette Blvd. from Sample Street to Stull Street. <br />Limitations: <br />• Utility lines will be shown based on field evidence located during our field survey and per <br />plans and marking provided by the owner, utility companies, or their representatives. We <br />cannot certify that all utilities required are available. <br />The estimated fee for performing the topographic survey will be $2,820.00. <br />412 5.
