SUBJECT: AT &T Authorization to Prepare Quote for Facility Relocation
Ignition Park South
This past year most utilities between Kemble and Scott streets have been relocated in
Ignition Park South, as well as the streets and alleys vacated and removed. As part of
the removal process of Kendall Street, an underground fiber line was discovered. This
line feeds a business on the north side of the Norfolk Southern Rail Road, and must be
relocated so this approximately 12 acre site can be fully utilized.
This line has been identified as being owned by AT &T, and the attached Authorization is
to allow them to engineer the relocation of their line and provide a cost estimate to
complete the work. AT &T estimates this work to range from $500 to $1,500 for a small
job.
While it's hoped this job would fall in to that category, because this is an active fiber line,
staff requests approval of the AT &T Authorization in a not -to- exceed of $5,000.
Once AT &T provides their work plan and cost estimate to actually reroute the line, staff
will provide the Commission with that information. At this point the only remaining utility
to relocate their lines within this area is AEP.

AT &T has received a request to perform the following Custom Work:

In accordance with the state tariffs, all expenditures incurred for quotation development and
engineering preparation for a special construction estimate are billable to the requestor. Once
the special construction estimate of charges has been developed, a contract with this
information will be provided to you for your approval. After the agreement to pay the special
construction charge is confirmed by returning the original signed contract, AT &T will schedule
the work to meet your request. The signed authorization needs be returned to the address
below before AT &T Indiana will proceed with the work.

Customer Authorization
I have requested and authorized AT &T to prepare a cost estimate associated with making
changes to AT &T's existing network on my behalf. I understand I am responsible for all
engineering costs. The anticipated engineering costs associated with this cost estimate could
range anywhere from $500 -$1500 (small jobs), however, I understand I will be billed for the
actual engineering time required to develop the cost estimate.
I further understand that if I proceed with the requested work, the engineering charges are
included as part of those costs.
