SUBJECT: Professional Services Proposal - Ziolkowski Construction Inc.
State Theatre façade terra cotta repair
This professional services proposal from Ziolkowski Construction Masonry Restoration is to
complete necessary terra cotta repair to the facade of the State Theatre (State). The
attached pictures show two damaged areas, which are limited to the southwest corner of the
building and above a second story window. Ziolkowski has done similar repairs to terra
cotta on the nearby American Trust Place's south façade, among others.
The State was at one time owned by the Commission, and when sold in 1993, a facade
easement was retained on the Michigan St. frontage to protect this iconic building. This
easement states the Commission agrees to maintain the west exterior façade (Michigan St.).
This repair is needed to stabilize the terra cotta, with an epoxy adhesive being used to
secure the fractured corner piece, and a patching material with applied finish to infill other
missing pieces.
Staff requests approval of this professional services agreement in the amount of$1,621, to
allow the repair and stabilization of these two areas of the State's facade.

Ziolkowski Construction will provide labor, equipment and material to perform the work as
follows:
Southwest Corner $1,326.00
1. Repair damaged terre cotta at the lower portion of corner with Jahn Stone Patch by
Cathedral Stone Company
2. Apply epoxy adhesive to shard and reset
3. Finish to match existing as close as possible
* Second Floor Window Head $ 295.00
1. Repair spalled terre cotta at window head with Jahn Stone Patch by Cathedral Stone
Company
2. Finish to match existing as close as possible
3. City of South Bend to provide boomlift
If Ziolkowski provides boomlift for repairs of second floor window head, add $512.00 for rental,
pickup and delivery.
