Subject: DBE Reevaluation Certification Letter
Congratulations!
We are pleased to inform you that your firm’s DBE Reevaluation Certification has been approved.
Your firm’s DBE/ACDBE certification applies only to the areas listed in the attachment to this letter. The
attachment also includes the NAICS Codes associated with each certified area of work.
Please read the following notice in its entirety, as it contains important information regarding your firm’s
continued certification.
If your firm acquires the resources and expertise, including equipment and personnel, to perform work in other
fields, please be advised that no DBE credit will be awarded for work performed outside your firm’s certified
areas or for work conducted prior to certification in those areas. A request to expand your firm’s certified areas
may require you to submit additional documentation and participate in an additional site visit. Conversely, if
your firm can no longer perform certain types of work or loses its pre-qualification status in any area, you must
notify INDOT of those changes within 30 business days.
To maintain your firm’s DBE status, you are required to notify INDOT immediately of any changes to your
firm’s location or contact information, including any updates to your email address. You must also inform
INDOT in writing of any change in circumstances affecting your firm’s ability to meet size, disadvantaged
status, ownership, control requirements, or any material change to the information provided in your application.
You must notify the certifier of a material change in your firm’s circumstances that affects its continued
eligibility within 30 days of its occurrence, explain the change fully, and include a duly executed Declaration of
Eligibility (DOE) with the notice. Failure to do so may result in removal of eligibility and/or suspension and
debarment.
A DOE form is required each year to verify that your firm continues to meet the standards of the DBE/ACDBE
program. The requirement to submit your firm’s annual DOE application is paused in 2026 and will
resume in 2027. The DOE will be available 60 days before the due date. Failure to complete your firm’s DOE,
and supply the necessary documentation, may result in the suspension of your firm’s eligibility. Your firm’s
DOE due date will not change due to your firm’s reevaluation certification.
INDOT reserves the right to initiate summary suspension or decertification proceedings if necessary.

On September 13, 2010, the Governor’s Commission on Supplier Diversity approved the department’s
effort to streamline its recertification process. Instead of conducting an onsite visit to each company seeking
recertification, the department now has the discretion to waive the visit after a thorough review of the
company’s file and recertification documents. We have approved your recertification and it is valid through
March 31, 2029. Please note that IDOA continues to reserve the right to conduct a site visit or phone
interview at any time with certified companies.
Although your certification is valid for three years, you are required to submit an annual Affidavit of
Continued Eligibility (ACE) form each year. Instructions on how to receive and complete this form can be
located at https://www.in.gov/idoa/mwbe/minority-and-womens-business-enterprises/certify-your-
business/. Please remember that you must notify us immediately if any changes occur. Failure to notify us
of changes or to provide an ACE form annually will result in the revocation of your certification. Changes
include, but are not limited to, changes in location, contact information, ownership, and control.
We encourage you to visit IDOA’s procurement website, www.in.gov/idoa/2464.htm, and update your
Business Registration Profile. You must review and update your profile regularly, because state purchasing
agents and prime contractors may use this information to contact you for business opportunities.
