Proposal is based on removal and replacement of an approximately 25'x17' pavement area. Pricing includes traffic control measures necessary to complete the work, including full road closure during the work. 2. Patch area shall be straight-line saw cut. Existing pavement shall be removed, visible subgrade conditions shall be evaluated, and base shall be brought to grade using #53 crushed concrete as needed within the proposed patch limits. Base shall be compacted to the extent practical based on existing field conditions, and 8" of asphalt shall be installed in accordance with COSB specifications. 3. Premium Services' scope is limited to the pavement patch area noted above. Any work outside the stated patch limits, including additional saw cutting, removal, excavation, base correction, asphalt, restoration, or traffic control beyond the proposed limits shall be additional cost. 4. Any additional patch size, additional asphalt depth, failed base/subgrade, unsuitable material, rework, or additional mobilizations will be completed by change order or T&M. 5. Existing pavement failure appears to be related to a previous utility repair/trench area with existing settlement and sinkhole conditions. Premium Services is not responsible for ongoing or future settlement, rutting, cracking, sinkholes, or movement caused by existing trench backfill, underground utility settlement, unsuitable material, drainage issues, or any pre-existing base/subgrade conditions below the proposed patch. Any warranty provided shall be limited to workmanship of the asphalt placement only and shall not include settlement or failure caused by underlying conditions outside of Premium Services' control. Note: This proposal may be withdrawn if not accepted within 21 days. Acceptance of Proposal - The prices, specifications, and conditions outlined above are agreed to and accepted. Authorization is hereby granted to proceed with the work as specified. Payment will be made in accordance with the terms described. Payment terms are 30 days unless otherwise agreed upon. All materials will conform to the specifications provided. All work will be performed in a professional manner consistent with standard practices. Any changes or deviations from the specifications that result in additional costs will be undertaken only upon written authorization, and such costs will be billed as an extra charge beyond the estimate.
