The City seeks to enhance its ability to identify responsive and responsible bidders on all City public worksprojects by institution of comprehensive submission requirements in compliance with State law. Qualityworkmanship, efficient operation, safety, and timely completion of projects requires that all bidders meetcertain minimum requirements to be responsive and responsible bidders.THIS FORM MUST BE SUBMITTED WITH YOUR BID.
"THIS FORM ONLY APPLIES TO BIDS GREATER THAN $250,000.
INSTRUCTIONS:
If you are a pre-qualified bidder, complete Section I only.If you are not a pre-qualified bidder, complete Section II only.Section II acts as an application for pre-qualification. Submission of Section II will allow the bidder to beconsidered for pre-qualification for bids with the City of South Bend Department of Public Works. Pre-qualified bidders will then be exempt from a portion of the submission requirements outlined in Section 6-71 of The Responsible Bidding Ordinance No. 10975-23 (hereinafter, "Responsible Bidding Ordinance")for a period of twelve (12) months.
Thereafter, contractors who are pre-qualified must submit a complete application for continuation of "pre-qualified" standing, on a form provided by the City ("Responsible Bidder Checklist (1) Pre-QualifiedBidders") within twelve (12) months of obtaining pre-qualified standing. If the status of any item changeswithin the twelve (12) months, it is the responsibility of the contractor to notify the City. Failure by any pre-qualified contractor to submit its complete application for continuation of "pre-qualified" standing within thetime prescribed above shall result in automatic removal of the designation, effective immediately followingthe twelve (12) months of pre-qualified standing.However, the "removed" contractor or subcontractor shall still be permitted to bid on City public worksprojects, though the contractor must submit all required documents under 6-71 until "pre-qualified" statusis re-established.
Please Note: The City reserves the right to request supplemental information from the bidder,additional verification of any information provided by the bidder, and may also conduct randominquiries of the bidder's current and previous customers regardless of pre-qualified standing.
It is the sole responsibility of the potential bidder to comply with all submission requirements applicable tothe bidder in Section 6-71 of the Responsible Bidding Ordinance no later than the date of the public bidopening.

POST BID SUBMISSIONS:
Post-bid submissions must be submitted in accordance with Section 6-72 of the Responsible BiddingOrdinance. The post-bid submission requirements are as follows:
1. All bidders shall collect, maintain, and provide upon request, a current written list that discloses thename, address, licensing status, and type of work for any subcontractor from whom the bidder hasaccepted a bid and/or intends to hire on any part of the public work project, including individualsperforming work as independent contractors.
2. Each subcontractor, whose portion of the project is estimated to be at least two-hundred fiffythousand dollars ($250,000.00), shall be required to adhere to the requirements of Section I of theResponsible Bidder Ordinance as though it were bidding directly to the City, except that thesubcontractor shall submit the required information (including the name, address, and type of work)to the successful bidder prior to the commencement of work.3. Failure of a subcontractor to submit the required information shall not disqualify the successfulbidder from performing work on the project and shall not constitute a contractual default and/orbreach by the successful bidder. However, the City may withhold all payment otherwise due forwork performed by a subcontractor, until the subcontractor submits the required information andthe City approves such information.
4. The disclosure of a subcontractor list ("Disclosed Subcontractor(s)") to the City by a bidder shallnot create any rights in the Disclosed Subcontractor(s). Thus, a bidder may substitute anothersubcontractor for a Disclosed Subcontractor by giving the City, upon request, written notice of thename, address, licensing status, and type of work of the substitute subcontractor.5. The successful bidder for projects greater than $250,000 and all subcontractors performing workgreater than $250,000 on a public works project are required to submit certified payroll utilizing thefederal form known as WH-347 or a similar form on a bi-weekly basis, submitted within 10 daysafter the end of each bi-weekly payroll period. Certified payrolls shall identify the iob title and craftfor each employee. Certified payrolls shall be submitted electronically.
Please Note: Submissions deemed inadequate, incomplete, or untimely by the City may result inthe automatic disqualification of the bid.
The City, after review of complete and timely submissions, shall, in its sole discretion, after taking intoaccount all information in the submission requirements, determine whether a bidder is responsive andresponsible, and provide a Pre-Qualification Verification Letter. The City specifically reserves the right toutilize all information provided in the contractor's submission and any information obtained by the Citythrough its own independent verification of the information provided by the contractor.

XXXVII CONTROLLED DEMOLITION FOR
RESEARCH MEASUREMENTS
1 GENERAL
A. Coordinate demolition activities with Own er-retained researchers performing measurementsduring controlled impact of designated building elements.
B. Controlled Impact Events consist of plaru'ied contact between demolition equipment andspecific building elements for research purposes.
2 COORDINATION
A. Conduct a pre-impact coordination meeting with Owner, Architect, Contractor, andResearcl'iers at least 14 days before first Controlled Impact Event.
B. Incorporate Controiled Impact Events into demolition schedule based on eight (8) hours totalon-site researcher participation and prior to any significant demolition of structures. Weatherconditions for scheduled day(s) must be at least 20F with minimal precipitation and wind gustspeeds less than 30 mph. Provide minimum 72 horirs' notice to Researchers prior to eachControlled Impact Event, unless otherwise agreed in writing.
C. Provide Researchers reasonable access to site before, during, and after impact activities forinstallation, monitoring, and removal of instruments. Researchers may also remove selectmaterials from the site during or following demolition for iaboratory testing in coordination withtlie demolition contractor.
3 CONTROLLED IMPACT ACTIVITIES
A. Perform impacts only at locations and in sequences approved by Owner and Researchers.
B. Execute impacts at speeds, angles, and repetition reasonably requested by Researchers, withinlimits of safe operation and equipment capabiiiry.
C. Temporarily pause demolition as reasonably required to allow data collection or verification.
4 DEMOLITION SEQUENCING
A. Modify demolition sequencing as required to preserve instnumented elements until ControlledImpact Eveiits are complete.
B. Do not remove, alter, or damage instrumented elements prior to written release byResearcliers.
5 SAFETY AND QUALIFICATIONS
A. Operators perforining Controlled Impact Events sliall have minimun'i five years' experienceoperating similar demolition equipment.
B. Comply with Project Safety Plan and establish exclusion zones during Controlled ImpactEvents.
C. Suspend impact activities iinmediately if unsafe conditions develop or upon direction ofOwner, Architect, or Researchers.
6 LIABILITY AND INDEMNIFICATION
A. Contractor shall not be responsible for damage to designated building eiements resultingdirectly from approved Coritrolled Impact Events.
B. Contractor shail be responsible for damage to research instniments, adjacent properly, orinjury to personnel caused by negligent acts or failure to follow approved procedures.

KLF Enterprises proposes to perform the full structural demolition of tlie cliurch structurelocated at 749 Harrison Street, South Bend, Indiana. The demolition will be completed in acontrolled, safe, and efficient maruier using industry-standard practices, specialized heavyequipment, and comprehensive site management procedures
KLF lias previously completed demolition of a cliurch structure of similar size and constructiontype, as noted on our reference project list. Based on tl'iat experience, we are confident in orirability to execute and manage this project successfully.
2. Equipment & Demolition Method
Demolition operations will be performed using a Link-Belt 490 excavator eqriipped witliappropriate attachinents, which may include:
Hydraulic sliear
Concrete processor
Bucket and tliumb
Grapple attacliment
The structure will be taken down using a top-down, controlled mechanical demolitionapproach, ensuring stability of remaining structural elements at all times. Materials will bebrought down in sections to maintain control and reduce risk of collapse beyond intended workareas.

3. Dust Control
Dust suppression will be handled using a mist cannon dust suppression machine, not astandard hose system. This equipment provides:

More consistent airborne particulate control
Dust control measures will be active during all demolition and debris liandling operations tominimize off-site migration and maintain compliance with local environineiital requirements.
4. Site Security & Safety Controls
To ensure public and worker safety:
