of the zoning ordinances, building codes, safety codes, ADA or other
requirements of the City of South Bend, the County of St. Joseph, the
State of Indiana, or the United States Federal Government.
This certificate is good for one year from the date of issuance and
is effective from the date entered herein. Plans are on file and open for
public inspection at the office of the Historic Preservation Commission of

Important Information Regarding this Request
REQUIRED DOCUMENTATION AND SITE PLANS
The Historic Preservation Commission of South Bend and St. Joseph County cannot process an Application
without required documentation. Comprehensive documentation provides a complete understanding of the
project. Problems often occur during a project review or during the execution of the project when the
property owner, staff or commissioners are unclear as to project specifics. Applications will NOT be
processed without required documentation.
Photographs may be submitted in digital format or in photographic print. When a project involves
blueprints and/or site plans, one (1) set shall be submitted with the Application. Any documentation
submitted to this office cannot be returned to the applicant.
APPLICATION FEE
The application processing fee for any Application for a Certificate Of Appropriateness is $20
Payment must accompany the application at the time of submission. For an electronically submitted
application, payment must reach the HPC office within 48 hours following transmission. Payment can be
made by credit card by contacting
Business License Administrator Michelle Adams at (574) 235-5912; by cash in person in the Department of
Community Investment on the 14th floor of the County City Building; or by check made out to City of
South Bend. Applications will NOT be processed without the required fee.
MEETING ATTENDANCE
When an Application has been scheduled for any meeting, the owner, architect or contractor(s) retained for
the project shall attend the meeting where their project will be reviewed and a decision rendered. Failure by
the owner, architect, or contractor to attend such meetings may result in denial of the Application due to
insufficient representation.
CERTIFICATE ISSUANCE
Certificates of Appropriateness will be filed with the Building Department of South Bend and St. Joseph
County. A copy of the Certificate will be emailed directly to the owner and/or contractor(s) listed on the
application.
INSPECTION AUTHORITY
All projects will be inspected during and following execution for compliance with the decision(s) rendered
by the Historic Preservation Commission of South Bend and St. Joseph County.

Owner acknowledges that while the Historic Preservation Commission only considers Certificates of
Appropriateness for exterior features, under certain circumstances it may be necessary for the Commission
Staff to have access to the interior of the building in order to accurately assess the condition of the exterior
feature and that the lack of access to the interior may prevent the Commission Staff from making a
favorable recommendation.
Any work performed on a historic landmark or in an historic district which does not conform to the
Certificate of Appropriateness certificate shall be immediately halted by the Historic Preservation
Commission and the Building Department of South Bend and St. Joseph County. Fines may be levied for
projects not in compliance.

Description of Proposed Work:
The proposed work consists of a complete replacement of approximately 2,200 square feet (22 squares)
of roofing in accordance with manufacturer specifications and industry standards. This includes the full
removal of the existing EPDM roofing system, including the membrane, flashings, and any deteriorated
components. A new 1-inch ISO insulation board will be installed and mechanically fastened, followed by
the installation of a new fully adhered TPO roofing membrane. All perimeter metal drip edge will be
replaced, and two new pipe boot flashings will be installed. The project also includes flashing and counter-
flashing of three chimneys, with brick repair on the east chimney to address any missing or damaged
bricks. Up to ten sheets of OSB decking will be replaced as needed, with additional OSB replacement
performed if required. All work will be completed to ensure a durable, watertight roofing system.

Terms
Terms and Conditions –
Financing: Financing up to $50,000 is available for all residential projects and provides multiple term options for
customers. Monthly payments are approximated without review of customer credit and may be subject to change
following a soft credit pull from the outside lender. Financing is approximated to be approved for all consumers
with a minimum credit score of 500.
Scope of Work: Company will provide services as described in the attached estimate. The company will provide
all services, materials, labor, tools, and equipment needed for completion of services unless requested from
customer to use specific materials purchased by the customer.
Payment Terms: For projects over $20,000, a draw schedule will be in place:
60% due upon acceptance of estimate and prior to scheduling of work
Balance due upon project completion
Unless otherwise agreed upon in writing by the project consultant and customer.
Credit card payments are subject to a 5% price increase due to processing fees.
Financing agreements are subject to a 3% price increase due to processing fees.
A $35.00 fee applies for each dishonored bank check.
Late Payment Fees: Past due balances are subject to a 5% late charge for the first 15 days, plus a $5.00 per
calendar day late fee beginning on the first day after the invoice due date. These fees will continue to accrue until
the balance is paid in full.
If full payment is not received within 60 days of project completion, a lien will be placed on the property in
accordance with Indiana law. The customer will also be responsible for all legal, court, and collection costs.
The date of project completion is defined as the date on which Reliable Roofing & Construction issues the final
invoice.
Change Orders: Any deviation from the original scope of work or cost must be documented with a written change
order signed by both Reliable Roofing & Construction and the Customer. Work performed by unauthorized
subcontractors without company approval will void the warranty.
Changed Conditions: Any hidden, unforeseen, or regulatory changes requiring additional work not covered in the
original scope will be subject to a change order and full payment by the Customer.
Warranty: Company warrants all work to be performed in a good and workmanlike manner. All materials are
subject to the manufacturer’s warranty terms. Extended warranties are available for an additional cost. Claims
must be submitted in writing. Company will respond in writing within 21 days with a resolution or dispute.
Proposal Validity: This proposal is valid for 4 weeks. If not accepted within this period, Reliable Roofing &
Construction reserves the right to revise or withdraw the proposal.
Accessories & Liabilities:
We are not liable for loss of satellite signal due to roof work.
We are not responsible for pre-existing gutter damage.
We cannot guarantee salvageability of gutters when multiple roof layers are present.
We are not liable for job site debris such as nails that may cause harm.
Roof drain replacement requires the customer to have their plumbing inspected prior to installation. Reliable
Roofing & Construction is not responsible for subsequent plumbing issues.
Customer Responsibilities:
Homeowner must maintain appropriate insurance coverage (fire, tornado, etc.).
Unless explicitly refused, Customer consents to the use of photos/videos of their home or property for marketing
purposes.
Safety and Conduct: Reliable Roofing & Construction reserves the right to terminate or suspend work if a
Customer, their representative, or anyone on their behalf exhibits threatening, abusive, or harassing behavior
(verbal or physical) toward any employee or subcontractor. In such cases, Reliable Roofing & Construction may
seek legal remedies and recover any unpaid balances due under the contract.
Weather Delays: Reliable Roofing & Construction is not responsible for project delays caused by inclement
weather, natural disasters, or other events beyond our control. In such cases, the project schedule will be
adjusted accordingly, and no penalties will be applied to either party.
Permits and Approvals: The Customer is responsible for securing any necessary HOA, municipal, or zoning
approvals unless otherwise specified in writing. Reliable Roofing & Construction will assist with permit
applications if agreed upon but is not responsible for permit delays or denials unless otherwise stated.
Right to Photograph Completed Work: Reliable Roofing & Construction reserves the right to take photos/videos
of completed work for documentation and marketing purposes. Customers may opt out by providing written
notice prior to project start.
Material Availability: In the event that specified materials are unavailable due to manufacturer delays or supply
chain issues, Reliable Roofing & Construction reserves the right to substitute with materials of equal or greater
quality, with Customer approval.
Dispute Resolution: Any disputes not resolved through mutual agreement shall be subject to mediation in St.
Joseph County, Indiana, before any legal proceedings are initiated. Both parties agree to share the cost of
mediation equally.
