CITY MOVING PERMIT APPLICATION CHECK LIST
LYKOWSKI CONSTRUCTION, INC. (herein LCI)
Prior to applying for a Moving Permit, both a Demolition and Foundation Permit must be obtained:
LCI's response: Prior to "issuing" a Moving Permit, both a Demolition and Foundation Permit must
be obtained.
There will be some demolition, however, the Owner has not made up their mind on which
Demolition Contractor will be performing this task.

'9 LCI will provide a COI with the specified coverage amounts naming the City of South Bend as co-
insured once the Move Permit has been approved and prior to issuing the Move Permit.
The City's required limits are not what we normally carry (due to the cost of the premiums which
are considerably higher than other Contractor's) and only increase our coverage when working in
South Bend. This practice has always been approved in our 52 years of business.

Good morning,
I agree, there are no overhead lines along the route that you will be moving the structure.
There are however two AEP owned streetlights along the route. If you can work around those
two minor obstructions, I don't believe I&M will need to be involved with this project.
