We've hosted large North Shore Triangle Neighborhood Association (NSTNA) events at
• Parkovash Park and Nokomis Park in the past (annual meetings, Halloween gathering, 4th
of July Bike Parade and Social, Movie Nights). However, this event is expected to have
interest beyond the neighborhood given the popularity of the featured band.

Parkovash Band performs rock and roll, blues, etc.. Members: Greg Helmen, Guy Luckert,
Andy Peck, Jimmy Rekos, and John Wiseman. Neighborhood musicians (warm-up acts)
include Scott Kirner, John Wiseman, Paul Costello, Skip Bean, and Juliana Ghourmley.
Sound checks 12-4p, warm-ups 4-5p, main band 5-8p. "Stage 20x24ft provided by SBVPA.

For each of the following, please provide detailed descriptions. If you run out of space, attach a response to this

Emergency Safety Plan - This plan should include, but is not limited to:
o The number of Indiana Law Enforcement Academy certified officers, fire, and emergency
medical personnel, and the need to use any of the City's public safety or emergency response
services.
o If hiring a security service, provide contact information and the number of hired event personnel.
o Proposed internal communications systems and public address systems.
With the COSB support we are closing streets and intersections adjacacent to the park to allow for
safe crossing and buffering of the event venue. We are also strategically placing the stage, picnic
tables, and food trucks in a manner will also buffer the park area. We will rely on COSB public safety
and emergency response services— diall 911. We have several American Red Cross, nurses, fire,
EMT, and police personnel that reside in the neighborhood, who are able to attend the event and to
provide assistance as needed. Public address capabilities via a stage microphone and speaker
system. We will establish cell phone contacts between key persons.
Proposed Cleanup Plan - This plan should include, but is not limited to:
o Measures in place to collect and remove trash, litter and recyclables.
We have 1 permanent trash bin with replacement contractor bag liners. We have requested 10 trash
boxes and liners from SBVPA. Adjacent neighbors to the park will loan their recycle containers for
the event. Voliunteers will remove litter from the park. " We will have a porta-potty loaned from the
• nearby Unity Garden. It will arrive on Friday, August 22 and be removed on Friday, August 29.
We've asked them to use weighted bags and not do any staking.
• . Inclement Weather Plan - This plan should include, but is not limited to:
o Safety measures that will be taken in the event of a tornado warning, tornado watch,
thunderstorm, and extreme temperatures.
We will cancel the event if forecasts predict or conditions create dangerous or unsafe weather. We
have the ability to notify people via a neighborhood -wide email list for the NSTNA, our Facebook
page, NextDoor app, and Parkovash Band's Facebook page. We can also post cancellations on
local media, signage at the park and signage in the neighborhod. We will have water stations
available. We will assign a 2 persons to track weather alerts leading up to and during the event.
We will act swiftly to vacate the park and to remove or secure equipment (tables, stage, etc.).
Proposed Lost & Found: We will have a volunteer -staffed NSTNA tent. We can make
announcements from the stage. We are able to provide post event communications as needed to
reconnect items or small children (Facebook, NextDoor, NSTNA email list, Hub kiosk at park).
Proposed Lost and Found Plan - This plan should include, but is not limited to:
o A description of the use of signage, announcements on public address systems or pre -event
handouts.

Provide an attached map with the geographic locations of all event items listed below.
Outline of entire event venue including the names of all affected streets and areas.
• Clear markings for street closures and a schedule for each.
• Location of fencing, barriers, and/or barricades. Indicate any removable fencing and exit locations for
emergency purposes.
• Location of all stages, platforms, bleachers, grandstands, tents, booths, cooking areas, vehicles, trailers,
and other temporary structures. Applicants should also clearly mark locations of food and alcohol

• The location(s) and number of all trash and recycling containers, including dumpsters.
• The location of generators or any source of electricity.

If you are using and/or closing public sidewalks or streets, you are required to notify area business owners and
residents. You must:
o Present your event concept to the surrounding stakeholders (residents, businesses, and neighborhood
groups) that represent the venue area.
• Attach a copy of the brochure or door hanger distributed to all affected residents/businesses/neighborhood
groups describing the event purpose, date and time.
Section J - Insurance
A Certificate of Insurance (copy) confirming the existence of a liability policy (General Liability and Automobile
Liability) of not less than $700,000 per occurrence and $1,000,000 aggregate, which specifically names the City
of South Bend, IN as an additionally insured for the event must be submitted. Copy of Certificate of Insurance
must be submitted two (2) weeks prior to the date of the event.

Section L - Permit & Agreement
1.
Pursuant to Local Ordinance No. 10628-18, there is a $50.00 non-refundable fee for Tier II and III event
applications filed 60 or greater days in advance of the event, or a $100 non-refundable expedited fee for
applications filed between 30 and 59 days in advance of the event.
2.
The APPLICANT must comply with all terms and conditions of this Permit and Agreement.
3.
The APPLICANT must obtain signatures from and/or make an attempt to notify all residents that reside
on the block. A copy of a brochure or door hanger distributed to all affected residents/businesses
describing the event purpose, date, time and contact information must be included with the
attachments to this application. The applicant is responsible for providing affected residents and
business owners with transportation to their property.
4.
The APPLICANT shall reimburse the Board for the actual cost to the City for the event, if deemed
necessary.
5.
Notification of approval/denial of this request will be issued by return of this form, upon signed
authorization by the Special Events Committee.
6.
The APPLICANT shall provide to the Board a Certificate of Insurance showing a liability policy in full
force and effect with limits of $700,000.00 per occurrence and $1,000,000.00 aggregate and the City of
South Bend, Special Events Committee, and Board of Public Works listed as an additional named insured
for this event.
7.
The APPLICANT assumes f ull responsibility for providing ample disposal containers for refuse/recycling
and assures the area will be cleaned up upon the conclusion of the event.
8.
Barricades will be delivered and picked up at the event location. The APPLICANT is responsible for
seeing that all cones are maintained and returned undamaged.
9.
The APPLICANT will follow the City of South Bend Noise Ordinance, which is in effect at all hours.

Between the hours of 11:00 p.m. and 7:00 a.m. certain noises are particularly prohibited. These include
operating radio receiving sets, musical instruments, phonographs and other sound reproduction devices if
audible fifty (50) feet away, as well as shouting, yelling, hooting, whistling, or singing in the streets in a
manner to disturb the peace (Municipal Code 13-57).
10. The APPLICANT assures the City that the area will be closed during the times indicated on the
application only. Event end times are pursuant to the recommendations of the South Bend Police
Department.

• Section F - Permit & Agreement
1. Pursuant to Local Ordinance No. 10628-18. there is a $25.00 non-refundable fee for applications filed 30
days or greater in advance of the event date. Applications filed less than 30 days in advance of the event
date will not be accepted.
2. All residents within the affected area must be notified of this event. The APPLICANT must obtain
signatures from at least 10 residents that reside along the closed right-of-way and make an attempt to
notify all other affected residents. APPLICANTS must include a copy of a brochure or letter of invitation
distributed to all affected neighbors describing the event purpose, date, and time.
3. The APPLICANT is responsible. prior to the event, for determining if there are any affected residents that
need assistance accessing their residence. The APPLICANT is responsible for providing said resident(s)
access or transportation to their property.
4. The cones will be delivered to the APPLICANT's address. The APPLICANT assumes full responsibility for
clean-up and assures the City that all cones will be maintained and returned undamaged. The APPLICANT
will be liable for the replacement cost of $50.00 per cone as a result of any missing or damaged cones.
5. Block parties must end by 8:00 p.m.
6. A street will be blocked off from intersection to intersection only. No half -blocks or alleys can be blocked
off.
7. The Special Events Committee reserves the right to deny any block party application based on traffic and

8. The Special Events Committee reserves the right to deny any block party application based on information
gathered from the South Bend Police Department or other sources.
9. The APPLICANT agrees to allow residents that live on the above -referenced block access in and out of the

12. The City of South Bend Noise Ordinance is in effect at all hours. Between the hours of 11:00 p.m. and
7:00 a.m. certain noises are particularly prohibited. These include operating stereos, speakers, musical
instruments, and other sound reproduction devices if audible fifty (50) feet away, as well as shouting,
yelling, hooting, whistling, or singing in the streets in a manner to disturb the peace (Municipal Code
13.57).

Backeround Information/The Vision
The North Shore Triangle Neighborhood Association was approached by a local70-80's cover
band, Parkovash, that originated in the North Shore Triangleto reunite and perform a free, early -
evening, family -friendly (no -alcohol) concert forthe neighborhood in Parkovash Park. The NSTNA
Board of Directors voted favorablyto pursue permission from South Bend Venues Parks and Arts
(SBVPA) to host a special event. And it seems based on the input of NST neighbors that there is
support for such an event. However, we would like to have yourfeedback specifically as a near -
neighbor to the park.
Schedule
12-4 pm Set-up (picnic tables and chairs, stage area/Boomer, trash receptacles, etc.)
4-6 pm Food Truck(s) availableforpick-up of pre -orders &walk-up orders
Begin street/intersection closures —full or partial.
4-5 pm Warm-up featuring musicaltalentfrom NST neighborhood.
5-8 pm Featured band "Parkovash" performs
8-9 pm Clean-up&event tear -down
Bathroom facilities will be available at the Unity Garden sight (Angela Hill), neighbors' own homes,
and any park -adjacent neighbor wishing to provide access (please notify
NorthShoreTrianpte(@gmaii.com if you are interested in this).
Special Considerations
We are requesting permission from the Board of Public Works to have some partial and some full
closures of intersections —intersections that are either adjacent or in close proximityto the park.
We need to take precautions against anyone attempting to drive into (or near) the park during the
event. We expect property owners and their vehicles will maintain access to their property
throughout the duration of the event. Inconvenience will be minimized as we maximize the safety
• of those attending the event. Here is what we are requesting:
