At its August 28, 2025 meeting, the Board of Public Works approved the above
referenced Change Order be increased by $5,800, bringing the revised contract amount
to $29,139.13.

Proposed Changes
Contract sum prior to change order was: $23,339.13
Change order: During a site visit on 8/8/25 Rebecca Maenhout had pointed out two areas of
sunken/deteriorated insulation where the clubhouse has been experiencing leaks, mentioning that she
would like those areas fully torn off and replaced. The two areas add up to approximately 500sgft and
the existing insulation is 6in. Below is our price to complete this change order.
Change Order Amount: $5,800.00
New Contract Sum Including Change Order will be: $29,139.13
This agreement shall be signed on behalf of A&M Home Services by Michael Albert
And on behalf f City of South Bend by Rebecca Maenhout
