At its May 13, 2025 meeting, the Board of Public Works approved the Final Change
Order for this project, be increased by $21,746 with an additional thirty-two (32) days. The final
contract amount is $500,746 with a completion date of August 15, 2024.
In addition, the Project Completion Affidavit for this project was approved in the amount
of $500,746. Copies of the Change Order and Project Completion Affidavit are enclosed for your
records.

The Division of Engineering and Division of Wastewater recommend the Board of Public Works
approval of Change Order No. 1 (FINAL), the Project Completion Affidavit, and closeout documents
for Project No. 123-031A WWTP Yard Piping Improvements.
Change Order No. 1 Summary
Additional Hydro Excavation to Expose Existing Utilities (Unforeseen Condition):
• Multiple subcontractors provided hydro excavation services to expose existing utilities in a
congested area between tanks. Many of the utility locations were unknown due to
inaccurate record drawings. Any of the utilities identified that were no longer in use were
removed during the excavation and hydro excavation work instead of adding additional
costs to protect or restore abandoned facilities.
• Cost Increase: $35,000.00
3-inch Reuse Water Line Repair:
• A 3-inch reuse water main ruptured during construction that required pavement removal,
excavation, repair and pavement restoration. This was completed by the contractor since
they were on -site and readily available to assist with the repair.
• Cost Increase $24,000.00
Adjustment in Quantities
• 6" WAS and Sludge Pipe Remove, reduced quantity by 167 LFT, Deduct $8,350.00
• 8" Sludge Pipe, Remove, reduced quantity by 2 LFT, Deduct $120.00
• 6" Ductile Iron Pipe, reduced quantity by 170 LFT, Deduct $28,220.00
• 8" Ductile Iron Pipe, reduced quantity by 5 LFT, Deduct $1,300.00
• 6" Ductile Iron 90 Deg Horizontal Bend, reduced quantity by 2 EACH, Deduct $11,000.00
• 8" Ductile Iron Transition Coupling, added quantity 1 EACH, Increase $9,000.00
• 5' Sidewalk, Concrete, 4 IN, reduced quantity by 32 LFT, Deduct $1,664.00

remain in full force and effect.
