The Preliminary Engineering Report (PER) is a crucial component of the DWSRF loan
application process. Its primary purpose is to provide a comprehensive evaluation of the
proposed project, ensuring it meets technical, financial, and environmental
standards. The PER helps the Indiana Finance Authority and other stakeholders
understand the project's feasibility and benefits, facilitating informed decision -making.
This PER consists of three project components. A PFAS minimization project at the City
of South Bend's North Water Treatment Plant, an improvement project at the City's
Edison Water Filtration Plant, and Lead Service Line Replacements in the City's
distribution system.
To accompany the PER for submission, the project team is requesting Board Signature
on the SRF Loan Program Signatory Authorization Resolution and PER Acceptance
Resolution.
MODEL PER ACCEPTANCE RESOLUTION
WHEREAS, the Cit of South Bend, Indiana, has caused a
Preliminary Engineering Report, PER, dated , to be prepared by the
consulting firm of ; and
WHEREAS, said PER has been presented to the public at a public hearing held
for their comments; and
WHEREAS, the Cit 's Board of Public Works finds that there was not sufficient
evidence presented in objection to the recommended project in the Preliminary
Engineering Report.
NOW, THEREFORE BE IT RESOLVED THAT:
The Preliminary Engineering Report dated
be approved and adopted by the
(City's) Board of Public Works; and
That said PER be submitted to the State Revolving Fund Loan Program for review
and approval.
Passed and adopted by the Cit 's Board of Public Works this 20th day of March, at
their regularly scheduled meeting.

SRF Loan Program
Signatory Authorization Resolution
Whereas, the City of South Bend, Indiana, (the "Participant") has plans for a Drinking Water
infrastructure improvement project to meet State and Federal regulations and the Participant intends to
proceed with the construction of such project:
Now, therefore, be it resolved by the Board of Public Works, the governing body of the Participant, that:
1. Eric C. Horvath, Director of Public Works, be authorized to make application for a State
Revolving Fund Loan ("SRF Loan") and provide the SRF Loan Program such information, data
and documents pertaining to the loan process as may be required, and otherwise act as the
authorized representative of the Participant; and
2. The Participant agrees to comply with State and Federal requirements as they pertain to the SRF
Loan Program; and
3. Two certified copies of this Resolution be prepared and submitted as part of the Participant's
Preliminary Engineering Report.
Adopted and Passed by the Board of Public Works, of the City of South Bend, Indiana, this
20th day of March, of 2025.
