AMENDMENT TO OWNER-ENGINEER AGREEMENT Amendment No. __2___ 1.Background Data: a.Effective Date of Owner-Engineer Agreement:October 10, 2023 b.Owner:Board of Public Works, City of South Bend, Indiana c.Engineer:United Consulting d.Project:Edison WTP Improvements, 123-067 2.Description of Modifications: [NOTE TO USER: Include the following paragraphs that are appropriate and delete those not applicable to this amendment. Refer to paragraph numbers used in the Agreement or a previous amendment for clarity with respect to the modifications to be made. Use paragraph numbers in this document for ease of reference herein and in future correspondence or amendments.] a. Engineer shall perform or furnish the following Additional Services: 1) Prepare State Revolving Fund (SRF) Preliminary Engineering Report (PER) sections for the Edison WTP to be incorporated into a PER by Others. 2) Prepare exhibits for alternatives. 3) Prepare environmental exhibits as required by IFA including wetland determination and flood plain boundaries from available federal information documentation. 4) Prepare cost estimates and assist with financial evaluations as required by the Indiana Finance Authority (IFA). 5) Coordination with IFA regarding the environmental assessment, preparation of letters of transmittal, cover letters, and responding to comments. 6) Final revisions to the PER following a review and comment period by Owner. 7) Assist with preparation for the public hearing including the agenda, attendance, and drafting of the meeting minutes. The responsibilities of Owner are modified as follows: 1) Provide asset management plan (AMP) information and documentation. 2) Provide information regarding the financial advisor / financial AMP section of the PER. 3) Assist in coordinating with other Engineers involved in the preparation of the overall PER, as needed. 4) Assist with communication with area residents, as needed. c. For the Additional Services or the modifications to services set forth above, Owner shall pay Engineer the following additional or modified compensation: 1) IFA SRF PER Preparation Assistance (Lump Sum): $39,200 d.
