The location will be the East side of Lafayette Blvd., North of Jefferson Blvd. Enclosed is the permit authorizing you to park your vehicle, KIA Sorento, in the corresponding parking space on the East Side of Lafayette Blvd., North of Jefferson Blvd. Please be advised that you must display the reserved handicap parking permit on your dashboard at all times, when you are parked in the corresponding reserved handicap parking space. Also, please note that this space is reserved as a handicap parking space only between the hours of 7:00 a.m. to 5:00 p.m., Monday through Friday. The fee for the Reserved Handicap Parking Permit will be Forty-Five Dollars ($45.00) per month, due by the first of every month, and made payable to the City of South Bend. Applicant Signature Date Typed or Written Name Board of Public Works Approval President Member Member Member Member Attest: November 26, 2024 1.Pursuant to Municipal Code Article 9, Section 20-113.1, any person eligible by virtue of a physical or mental disability that substantially limits one or more of the major life activities of such individual as defined by the Americans with Disabilities Act, shall present to the Board of Public Works a written application for a permit entitling that person to the exclusive use for a period of one calendar year, or less, from the date of issue of one parking space adjacent to or directly across the street from the applicant's place of employment. The application must be filled out completely. The cost per month is $45.00. 2.The application shall include a letter from a qualified physician on their letterhead verifying that the applicant is a physically disabled person. 3.The application for parking at a place of employment shall include evidence of approval by the employer as well as the information regarding the applicant's daily work schedule. No applicant shall be granted a permit without first demonstrating to the Board that the individual is a full-time employee. 4.The application must also provide verification from the individual's supervisor at their place of employment attesting to the fact that the employer has exhausted all efforts at providing parking for the individual. No applicant shall be granted a permit unless the applicant's employer provides the board with information that the applicant's employer is unable to provide the applicant with any other parking opportunities comparable to that which the City is able to provide in terms of distance from the applicant's place of employment. The burden is on the applicant and the applicant's employer to demonstrate to the Board that the applicant's employer is unable to provide the applicant with a parking area that adequately meets the needs of the applicant. 5.The application must be renewed on a yearly basis, with applications due to the Board of Works by December 1 of the previous year and all required documents attached. 6.Approval is subject to availability.
