1.Pursuant to Municipal Code Article 9, Section 20-113.1, any person eligible by virtue
of a physical or mental disability that substantially limits one or more of the major life
activities of such individual as defined by the Americans with Disabilities Act, shall
present to the Board of Public Works a written application for a permit entitling
that person to the exclusive use for a period of one calendar year, or less, from the
date of issue of one parking space adjacent to or directly across the street from the
applicant's place of employment. The application must be filled out completely. The
cost per month is $45.00.
2.The application shall include a letter from a qualified physician on their letterhead
verifying that the applicant is a physically disabled person.
3.The application for parking at a place of employment shall include evidence of
approval by the employer as well as the information regarding the applicant's daily

to the Board that the individual is a full-time employee.
4.The application must also provide verification from the individual's supervisor at
their place of employment attesting to the fact that the employer has exhausted

distance from the applicant's place of employment. The burden is on the applicant
and the applicant's employer to demonstrate to the Board that the applicant's
employer is unable to provide the applicant with a parking area that adequately
meets the needs of the applicant.
5.The application must be renewed on a yearly basis, with applications due to the
