At its October 8, 2024 meeting, the Board of Public Works approved the above
referenced Change Order be increased by $39,562.50, bringing the revised contract amount to
$1,802,562.50 with an additional thirty (30) days and with a new completion date of January 19,
2025.

Demolition of South Bend Medical Foundation
DATE: 09/30/2024 During the course of decommissioning the South Bend Medical Foundation to be prepared for demolition, additional costs were incurred for asbestos removal and universal waste disposal. The contract includes a $100,000 allowance for these specific items, but that limit was exceeded. The cost breakdown is as follows:
• Additional Asbestos Removal (unforeseen): $38,940.00 o This includes wall mastic that was hidden during the original asbestos survey and uncovered once wall coverings were removed.
• Additional Asbestos Removal (unforeseen): $13,172.50 o This includes transite panels that were hidden during the original asbestos survey and uncovered after removing exterior siding.
• Universal Waste Removal (anticipated cost): $74,250.00
o This includes removal of all universal waste found in the building: computers, batteries, monitors, fluorescent lights, mercury-containing equipment.
• Waste Oil / Sludge Removal (anticipated cost): $13,200.00 o This includes removal of all waste oil and sludge found in diesel tanks on the site.
• Total of all added costs: $139,562.50
• Apply existing $100k allowance: ($100,000)
• This change order: $39,562.50
