attention for Board of Public Works approval at lhensley&southbendin.gov :
1) One (1) signed original of the Contract (enclosed); and
2) Certificate of Insurance naming the City of South Bend as an additional insured.

quotes, using a simple quote process, from qualified firms for repairing the tree lawn and
sidewalk outside of Howard Park. 3 quotes were requested and 1 was received and opened.

RE: Howard Park Sidewalk Patch
Rebecca,
We propose to install a new sidewalk in the tree lawn area in front of the Howard Park Public
House delivery entrance, approximately 100 sf. Our quote includes:
• Mobilization.
• Removal and haul off 4" topsoil and approximately 4" of fill below.
• Import, place, grade, and compaction of 4" of # 53 stone.
• Expansion joint material with cap and caulking at existing curb and walk.
• Placement, finish, and cure of new Class A 4" concrete walk.
• Clean up and demobilization.
Exclusions / Clarifications:
• Lawn Restoration
• We will call 811 for public utility locates. Please have any private utilities located prior
to commencement of work.
• All work to be completed between the hrs. of 7:00 -3:30 Monday through Friday.
Total Price: $5,360
