found in Chapter 21 (Zoning), South Bend Municipal Code and Chapter
26 of the St. Joseph County Code.
The issuance of this certificate does NOT in any manner, release
the recipient from the responsibility of complying with the requirements
of the zoning ordinances, building codes, safety codes, ADA or other
requirements of the City of South Bend, the County of St. Joseph, the
State of Indiana, or the United States Federal Government.
This certificate is good for one year from the date of issuance and
is effective from the date entered herein. Plans are on file and open for

Payment Terms. All invoices for services covered by the estimate are payable upon completion of work or due
date stated on this invoice. Late Charges; Interest. Any invoices not paid by you on the date of completion may
bear interest after the due date until the invoice is paid. The accepted forms of payment are listed on this invoice.
Payment Terms: Credit card payments are subject to a 5% price increase due to processing fees. Financing
agreements are subject to a 3% price increase due to a processing fees. A down payment of 50% is due upon
acceptance of estimate. The balance of the contract is due the day of project completion. In the event of
nonpayment of balance within 10 business days of job completion, the customer must pay to Reliable Roofing &
Construction a late charge of 5% of the unpaid balance for the first 60 days. Any remaining unpaid balance after
60 days will be turned over to collections, and all collection and court costs will be paid by the customer. The
customer further agrees that it will be charged $ 35.00 for each dishonored bank check.

Tearing off includes, setting up the dumpster at a predetermined spot (preferably as close to the working
area as possible). Securing the sides of the house, and landscaping with tarps, and OSB/plywood to help
contain debri during the tear off and ensure a quick precise cleanup.
We then tear down the layer(s) of shingles, felt paper, and ice and water shield down to the decking/wood,
and check for rotted wood to be replaced.
