SECTION I EXPERIENCE QUESTIONNAIRE
1. Attach information regarding projects your organization has completed for the period of one
(1) year prior to the date of the current bid.
2. Attach a listing of public works projects currently in process of construction by your
organization.
3. Attach information regarding any failure to complete any work awarded to you and the
location thereof.
4. Attach references from private firms for which you have performed work.
SECTION II PLAN AND EQUIPMENT QUESTIONNAIRE
Attach an explanation of your plan or layout for performing proposed work. (Examples could
include a narrative of when you could begin work, complete the project, number of workers,
etc. and any other information which you believe would enable the City of South Bend to
consider your bid.)
2. Attach a listing of the names and addresses of all subcontractors (i.e. persons or firms
outside your own firm who have performed part of the work) that you have used on public
works projects during the past five (5) years along with a brief description of the work done
by each subcontractor.
3. If you intend to sublet any portion of the work, attach the name and address of each
subcontractor, equipment to be used by the subcontractor, and whether you will require a
bond. However, if you are unable to currently provide a listing, please understand a listing
must be provided prior to contract approval. Until the completion of the proposed project,
you are under a continuing obligation to immediately notify the City of South Bend in the
event that you subsequently determine that you will use a subcontractor on the proposed
project.
4. Attach a listing of equipment you have available to use for the proposed project.
5. Have you entered into contracts or received offers for all materials which substantiate the
prices used in preparing your proposal? If not, attach an explanation for the rationale used
which would corroborate the prices listed.
SECTION III CONTRACTOR'S FINANCIAL STATEMENT
Attachment of bidder's financial statement is mandatory. Any bid submitted without said
financial statement as required by statute shall thereby be rendered invalid. The financial

4. Contractor does not knowingly employ or contract with an unauthorized alien, nor retain any
employee or contract with a person that the Contractor subsequently learns is an unauthorized alien.

Contractor agrees that he/she/it shall enroll in and verify the work eligibility status of all of Contractor's
newly hired employees through the E-Verify Program as defined by I.C. 22-5-1.7-3. Contractor's
documentation of enrollment and participation in the E-Verify Program is included and attached as part of
this bid/quote; and
5. Contractor shall require his/her/its subcontractors performing work under this public contract to
certify that the subcontractors do not knowingly employ or contract with an unauthorized alien, nor retain
any employee or contract with a person that the subcontractor subsequently learns is an unauthorized alien,
and that the subcontractor has enrolled in and is participating in the E-Verify Program. The Contractor
agrees to maintain this certification throughout the term of the contract with the City of South Bend, and
understands that the City may terminate the contract for default if the Contractor fails to cure a breach of
this provision no later than thirty (30) days after being notified by the City.
6. Persons, firms, partnerships, corporations, associations, or joint venturers awarded a contract by
the City of South Bend through its agencies, boards, or commissions shall not discriminate against any
employee or applicant for employment in the performance of a City contract with respect to hire, tenure,
terms, conditions, or privileges of contract or employment, or any matter directly or indirectly related to
contracting or employment because of race, sex, religion, color, national origin, ancestry, gender
expression, gender identity, sexual orientation, or due to age or disability that does not affect that person's
ability to perform the work.
In awarding contracts for the purchase of work, labor, services, supplies, equipment, materials, or any
combination of the foregoing including, but not limited to, public works contracts awarded under public
bidding laws or other contracts in which public bids are not required by law, the City, its agencies, boards,
or commissions will consider the Contractor's good faith efforts to obtain participation by those
subcontractors certified by the State of Indiana as a Minority Business ("MBE") or as a Women's Business
Enterprise ("WBE") as a factor in determining the lowest, responsible, responsive bidder.
Contractors seeking the award of a City contract cannot be required to award a subcontract to an MWBE;
however, they may not unlawfully discriminate against said MBE/WBE. On goal -eligible contracts,
Contractors are required to either meet both MBE and WBE utilization goals or demonstrate that the
Contractor has made good faith efforts to obtain participation from MBE and WBE subcontractors. A
finding of noncompliance or a discriminatory practice shall prohibit that Contractor from being awarded a
City contract for a period of one (1) year from the date of such determination, and such determination may
also be grounds for terminating the contact to which the discriminatory practice or noncompliance pertains.
7. The undersigned Contractor agrees that the following nondiscrimination commitment shall be
made a part of any contract which it may henceforth enter into with the City of South Bend, Indiana or any
of its agencies, boards or commissions.
Contractor agrees not to discriminate against or intimidate any employee or applicant for employment in
the performance of this contract with privileges of employment, or any matter directly or indirectly related
to employment, because of race, religion, color, sex, gender expression, gender identity, sexual orientation,
handicap, national origin or ancestry. Breach of this provision may be regarded as material breach of

The City seeks to enhance its ability to identify responsive and responsible bidders on all City public
works projects by institution of comprehensive submission requirements in compliance with State law.
Quality workmanship, efficient operation, safety, and timely completion of projects requires that all bidders
meet certain minimum requirements to be responsive and responsible bidders.
THIS FORM MUST BE SUBMITTED WITH YOUR BID.
INSTRUCTIONS:
If you are a pre -qualified bidder, complete Section I only.
If you are not a pre -qualified bidder, complete Section II only.
Section II acts as an application for pre -qualification. Submission of Section II will allow the bidder to be
considered for pre -qualification for bids with the City of South Bend Department of Public Works. Pre -
qualified bidders will then be exempt from a portion of the submission requirements outlined in Section 6-
63 of The Responsible Bidding Ordinance No. 10594-18 (hereinafter, "Responsible Bidding Ordinance")
for a period of twelve (12) months.
Thereafter, contractors who are pre -qualified must submit a complete application for continuation of "pre -
qualified" standing, on a form provided by the City ("Responsible Bidder Checklist (1) Pre -Qualified
Bidders") by December 311t for the upcoming calendar year, or within twelve (12) months of obtaining pre -
qualified standing. If the status of any item changes within the twelve (12) months, it is the responsibility
of the contractor to notify the City. Failure by any pre -qualified contractor to submit its complete
application for continuation of "pre -qualified" standing within the time prescribed above shall result in
automatic removal of the designation, effective January 1 of the following year, or immediately following
the twelve (12) months of pre -qualified standing.
However, the "removed" contractor or subcontractor shall still be permitted to bid on City public works
projects, though the contractor must submit all required documents under 6-63 until "pre -qualified" status
is re-established.
Please Note: The City reserves the right to request supplemental information from the bidder,
additional verification of any information provided by the bidder, and may also conduct random
inquiries of the bidder's current and previous customers regardless of pre -qualified standing.
It is the sole responsibility of the potential bidder to comply with all submission requirements applicable to
the bidder in Section 6-63 of the Responsible Bidding Ordinance no later than the date of the public bid
opening.

POST BID SUBMISSIONS:
Post -bid submissions must be submitted in accordance with Section 6-64 of the Responsible Bidding
Ordinance. The post -bid submission requirements are as follows:
1. All bidders shall collect, maintain, and provide upon request, a current written list that discloses
the name, address, licensing status, and type of work for any subcontractor from whom the bidder
has accepted a bid and/or intends to hire on any part of the public work project, including
individuals performing work as independent contractors.
2. Each subcontractor, whose portion of the project is estimated to be at least one -hundred fifty
thousand dollars ($150,000.00), shall be required to adhere to the requirements of Section I of
the Responsible Bidder Ordinance as though it were bidding directly to the City, except that the
subcontractor shall submit the required information (including the name, address, and type of
work) to the successful bidder prior to the commencement of work.
3. Failure of a subcontractor to submit the required information shall not disqualify the successful
bidder from performing work on the project and shall not constitute a contractual default and/or
breach by the successful bidder. However, the City may withhold all payment otherwise due for
work performed by a subcontractor, until the subcontractor submits the required information and
the City approves such information.
4. The disclosure of a subcontractor list ("Disclosed Subcontractor(s)") to the City by a bidder shall
not create any rights in the Disclosed Subcontractor(s). Thus, a bidder may substitute another
subcontractor for a Disclosed Subcontractor by giving the City, upon request, written notice of the
name, address, licensing status, and type of work of the substitute subcontractor.
5. The successful bidder and all subcontractors on a public works project are required to submit
certified payroll utilizing the federal form known as WH-347 or a similar form on a bi-weekly basis,
submitted within 10 days after the end of each bi-weekly payroll period. Certified payrolls shall
identify the job title and craft for each employee. Certified payrolls shall be submitted
electronically.
Please Note: Submissions deemed inadequate, incomplete, or untimely by the City may result in
the automatic disqualification of the bid.
The City, after review of complete and timely submissions, shall, in its sole discretion, after taking into
account all information in the submission requirements, determine whether a bidder is responsive and
responsible, and provide a Pre -Qualification Verification Letter. The City specifically reserves the right to
utilize all information provided in the contractor's submission and any information obtained by the City
through its own independent verification of the information provided by the contractor.

indicates and ensures I have sufficient employees on staff to complete the work.
It outlines how I intend to meet the staffing needs of the work.
(iii) V List of projects of similar size and scope of work performed in all areas, including
the State of Indiana, within the last three (3) years.
(iv) V For every project, submit evidence of participation in apprenticeship and training
programs, applicable to the work to be performed on the project, which are
approved by and registered with the United States Department of Labor's Office
of Apprenticeship, or its successor organization.

Design -Build improvements on CTA's Red Line on the north side of Chicago. This project
is the largest capital improvement in CTA's history. Work includes train control systems
(signals, communications, and traction control) improvements from Belmont to Howard,
construction of a new "fly -over" structure to carry the Brown Line over the Red and
Purple lines just north of Belmont Avenue, as well as 0.3 miles of staged mainline track
structure replacement beneath the new fly -over bridge, and reconstruction of 1.3 miles
of new elevated track structure between Lawrence and Ardmore, as well as the complete

The Northeast Flow Control Facility (NE FCF), a flow control facility rated for 200 million

included working around existing utilities and process piping including live tapping into

My name is Helen Sookradge, and I am an Infection Preventionist with the Beacon Health System, more
specifically, with Memorial Hospital. I have been in Infection Prevention over ten years.
In April 2022, a new demolition and rebuild of our 6th Floor started. We had no idea the turn of events
that we would encounter along the way. Having to work in phases to restructure the plumbing system for the 6th
floor to align with both floors 5 and 7, and the coordination with Facilities for the numerous water shutdowns,
uncounted, resulted in a total of 24 PCRAs or Pre -Construction Risk Assessments and 24 ICRA permits that were
issued. I have to say that this is more permits for one project than I have ever had to issue in my career to date.
This project quickly became extremely complicated, this team with their subcontractors had to sister in the new
plumbing with the old. A great undertaking at best. Walsh always kept me apprised of the timelines, with new
drawings as the actual phasing diagrams changed multiple times weekly, and at times daily. Walsh had excellent
communication both via email and voice calls including text messaging as quick communication was needed at
times to either change an existing permit phase number, or to update and extend current permits. I must say it is
not always a pleasure to work with individuals under such pressure situations. However, that was not the case
with Walsh. They always communicated in a calm and precise manner. If Walsh did not have the answers needed,
they would seek out the answer and return that to me quickly.
Recently, another Walsh project has come about as a result of the Tower Project in which, due to the
excellent working relationship I had on the 6th Floor project with Walsh, I reached out to Don Gee, Director
Projects and Construction here at Memorial Hospital to ask if Walsh could be involved on this 3HV renovation
project. I was fortunate that Don was able to make this happen. I had said to Don at one point, "I do not know
what strings you pulled, but I am happy that you could make this happen." I have to say that Walsh is an asset and
has made my job as an RN, in Infection Prevention a joy in a potentially frenzied situation in the continuously
changing project that the 6th Floor project became. I want to say that I am truly grateful to have the opportunity to
work with such a caliber of company as Walsh. I know the project will be handled appropriately, and corrections
made immediately. Thank you.

Kishwaukee Hospital, part of Northwestern Medicine Network, hired Walsh Construction in September 2019 to
complete a renovation project within our existing main laboratory department. The project was completed in six phases
and included renovation to the laboratory department to facilitate new equipment and layouts.
Walsh was selected to construct this project based on best overall value. Construction in an operating hospital can be a
stressful process. Walsh, along with team members Eric Had€off (Sr. Project Manager) and Luke Oberhelman
(Superintendent), alleviated a lot of the stressors. They worked well with patients and staff to ensure they had as little
impact as possible, communicated well to all hospital departments involved in the project, kept competent staff onsite
to oversee the work and stood behind ensuring the initial budget and schedule were maintained throughout the course
of the project. The team was extremely professional, knowledgeable, responded promptly to questions, and kept the
job site clean and free of clutter. It was a real joy to work with them.

5. PRE -APPRENTICE PROGRAM
Walsh has created and implemented a Pre -Apprentice Program at a variety of projects throughout Chicago, including the Old Cook
County Hospital Redevelopment, the Red -Purple Line Modernization project, the Wilson Red Line Renovation project and the Lawson
House. The goal of this program is to teach basic technical and job -readiness skills for a designated occupation or occupational sector
that prepared participants for Registered Apprenticeship training. This program was in concert with Communities Empowered through
Construction (CEC) and The Target Group.
Benefits for Participants in the Pre -Apprentice Program:
• Hands on experience in the field of construction. Qualify to meet the minimum standards for a Registered Apprentice Program.
• Benefit from management and tradesmen training while building communication and work -readiness skills.
• Assistance in exposing participants to Registered Apprentice Programs to help advance into registered apprenticeship programs.
Program Overview:
• Week 1: Selected participants shadow Walsh management staff to get an overview of how construction projects are managed.
• Week 2: Selected participants shadow our superintendents and experience the different onsite trades and the scopes of work
being performed.
• Weeks 3-8: Boots to the ground. Selected participants are assigned to a specific trade, of his or her choice and shadow the
foreman who will train and delegate specific minimal work tasks for hands-on experience. During these weeks, participants will
also take training classes (OSHA 10-Hour; Flagging Certification; Fall Protection; Confined Space; First Aid, etc.).
• Week 8: Participants will present on their experience with the program and will receive a certificate of completion.
Program Selection Criteria:
• A resident of a target ward (refer to the map down below)
• Valid Driver's License
• Meet physical requirements to perform construction work
• Pre -drug screening
• Must be at least 18 years old with a High School diploma or equivalent
• Self-motivated/willingness to learn with a positive attitude

OLD COOK COUNTY HOSPITAL REDEVELOPMENT
How Walsh Incorporated Diversity & Inclusion Plans: At the redevelopment of the old Cook County Hospital, Walsh
created and implemented a Pre -Apprentice Program. Each Pre -Apprentice from the first class was sponsored by a
subcontractor into a union. They later returned to work as apprentices on the Cook County jobsite.
Results: Walsh exceed the initial goals by achieving 37% MBE participation and 15% WBE participation. The original
goals were 30% for MBE participation and 10% for WBE participation.
!V1 RED LINE WILSON STOP
How Walsh Incorporated Diversity & Inclusion Plans: The planned reconstruction of the Wilson Red Line station
began in 2014, and replaced the badly deteriorated facility that was built in 1923. The completely rebuilt, modern and
accessible station serves as a new transfer point between Red and Purple Line Express train service.
Results: Walsh met the DBE goal of 27.5%. The station opened to the public ahead of schedule in September 2017 even
though the Wilson Station Reconstruction Project was one of the largest CTA'L' station projects in the agency's history.
RED -PURPLE LINE MODERNIZATION
How Walsh Incorporated Diversity & Inclusion Plans: Despite many opportunities for internships and jobs that
offer stable careers and growth, the Chicago Transit Authority (CTA) observed there was a disproportionate lack of
candidates from economically disadvantaged and minority neighborhoods. In response, the Walsh -Fluor joint venture
team that worked on the Red -Purple Line Modernization project teamed up with the CTA and Chicago Public Schools
to create the CTA Elevating Futures Scholarship Fund.
Results: The new program targeted STEM -focused Chicago high school students that expressed interest in the
pursuit of careers in engineering or construction. Walsh -Fluor committed to contributing $250,000 towards the new
scholarship, which will award three students with $25,000 in each of the programs first two years. In the final two years
of the fund, two students will be awarded the scholarship.

Subject: Alpine Demolition Services, LLC (Alpine) demolition plan of the existing buildings at the above location.
1. Existing Conditions:
• 7 story building with multiple additions and a basement.
• Standalone chiller Enclosure
2. Safety and Site Security:
• All personnel on site have been trained by Alpine Demolition on safety and OSHA rules regarding demolition.
• Proper PPE to be worn by all personnel on site.
o Hard hats, safety glasses, highly visible safety vests, steel toe work shoes.
o Dust masks or half face respirators in dusty conditions.
3. Mobilization/Construction Access:
• Equipment will be delivered via semi low boys and stored on site within the work area for the duration of the
specific demolition activities.
• All deliveries will access the jobsite via the assigned gate.
• Access to the work zone to be limited to Construction personnel only.
4. Traffic Control
o Excavator and skid steer will be kept on site within project limits. Dump truck loading will be handled within project
limits. Flagman will be present when trucks are entering and leaving the job site.
5. Emergency Response:
• Emergency services on this project can be accessed by 911
• Clinic: TBD
• All personnel to sign in at beginning of shift.

7. Protection during demolition:
• Chain link fence to be set up around the perimeter.
• Signs to be posted on fence "Danger do not enter" by Alpine Demolition.
• Alpine Demolition will spray water on demolition debris to keep dust down.
• Spotters will be utilized during demolition preventing pedestrians from entering the demolition zone.
• Site to be left clean at end of shift with no loose debris.
8. Engineer Controls
o Alpine demolition will hook up to a hydrant to utilize water for dust control.
o Garbage trucks will cover the trailer prior to transport out of the work area to prevent spillage.
9. Equipment:
• Volvo EC750E Hi Reach Excavator w/ Hydraulic Attachments
• CAT 349 Hydraulic Excavator w/ Hydraulic Attachments
• CAT 262 skid steer with grapple and sweeper attachment
10. Building Demolition
o Temp fence and erosion control measured are installed
o Conduct OSHA Engineering Survey
o Mobilize ACM contractor onsite and begin abatement.
o Once all ACM have been removed from an area, universal waste will be collected and disposed of.
o Interior strip out of the building will commence, utilizing interior shafts as debris chutes.
o Debris will be removed as it demoed to reduce accumulation of materials onsite.
o Once Asbestos Abatement and Interior strip out has been completed in a portion of the building, structural
demolition will begin.
o Starting from the south, Excavators will begin demoing the building using hydraulic attachments.
o Building materials will be separated onsite and loaded into trucks for disposal.
o All debris will be sprayed with a fire hose throughout the process to reduce dust.
o Trucks to be loaded within work area for removal of material off site per contract specifications.
o Foundations and slab will be processed and loaded offsite as demolition progresses.
o North and East foundation walls to remain in place to support adjacent grade.
o Backfill to be brought to the site, compacted per specs, and fill basement voids.
o Affected streets to be swept clean.

Walsh Construction maintains a flexible staffing approach, with over 8,000 employees nationwide and 200 healthcare -dedicated
management professionals. Many team members are cross -trained in multiple management roles if the validation phase extends
or preconstruction is delayed. Walsh Construction is fully able to allocate the right staffing resources to this project. If the award
timeframe changes, we will adjust staff assignments accordingly. If necessary, Walsh has the ability to increase onsite staff due to
the current Beacon Memorial Campus redevelopment in South Bend.
Walsh managers are directed to maintain continuous communication with subcontractors. Project changes, RFI and submittal
deadline reminders, and other notices are shared constantly with subcontractors to ensure that they are aware and on top of their
responsibilities. We listen to and work through subcontractor issues to minimize schedule and cost impacts.
