SECTION II PLAN AND EQUIPMENT QUESTIONNAIRE 1. Attach an explanation of your plan or layout for performing proposed work. (Examples could include a narrative of when you could begin work, complete the project, number of workers, etc. and any other information which you believe would enable the City of South Bend to consider your bid.) 2. Attach a listing of the names and addresses of all subcontractors (i.e. persons or firms outside your own firm who have performed part of the work) that you have used on public works projects during the past five (5) years along with a brief description of the work done by each subcontractor. 3. If you intend to sublet any portion of the work, attach the name and address of each subcontractor, equipment to be used by the subcontractor, and whether you will require a bond. However, if you are unable to currently provide a listing, please understand a listing must be provided prior to contract approval. Until the completion of the proposed project, you are under a continuing obligation to immediately notify the City of South Bend in the event that you subsequently determine that you will use a subcontractor on the proposed project. 4. Attach a listing of equipment you have available to use for the proposed project. 5. The building footprint plus any damaged asphalt should be considered the limits of demolition. 2. There are erosion control requirements in the spec. It states that we are required to develop and follow an erosion and sedimentation plan and submit periodic inspection reports. What are the requirements for this plan? a. Contractor shall follow the general requirements outlined in IDEM's Indiana Storm Water Quality Manual. The disturbance is below the threshold requiring a Construction Stormwater General Permit (CSGP), however it is incumbent upon the contractor to maintain a "clean" site and prevent the discharge of any pollutants (sediment, chemical, or otherwise) from the site. Contractor shall be responsible for reporting any spills as applicable. 3. In regard to erosion control, nothing is shown on the drawings. Is the contractor simply required to protect the storm drains with inlet protection as required and install sit logs around the perimeter fence to ensure no run-off occurs? What other requirements are there? a. See response to question above. Contractor shall be responsible for any applicable measures necessary to prevent sediment runoff from the site. This typically occurs via silt fence or silt logs around the perimeter (based on surface type) and inlet protection measures to prevent runoff from entering the storm sewer system. In addition, any land that is left undisturbed for more than 7 days shall have temporary or permanent (depending on stage of construction) seeding applied to prevent sediment runoff. 4. The documents state that fuel/oil tanks that feed the generators are to be removed. Is there decommissioning required on these fuel tanks? Where are they located? a. The tank is above ground just south of the building on the west border of the property. Unknown if decommissioning is required. 5. Do you have any reports on the UST? a. No. 6. During the last walk, the owner was mentioned as potentially completing some refrigerant recovery. Please clarify what refrigerant will be left on -site for the contractor to recover and what will be removed prior to the contractor arriving onsite. a. American Axle & Manufacturing Detroit Manufacturing Complex Decommissioning, Decontamination & Demolition This project, located in Detroit, Michigan was for the sale of all assets, plant decommissioning, decontamination and demolition of the 1.6M sf manufacturing facility. The work included asbestos abatement, universal waste removal, demolition of multiple structures of varying heights and the installation of new perimeter fencing to secure the site. Adamo purchased the project from the Owner, exclusive of the land. There was a fee paid to the Owner at the outset of the project with a guarantee of completion with no fees to the Owner. The project produced very little in the way of debris. All non-ferrous metals, 2.5M pounds and ferrous metals, 30,000 tons, were recycled and the concrete produced by the demolition was crushed onsite by Adamo and used as fill in machine pits, trenches and other voids on the site.
