The APEX Consulting and Surveying, Inc. (APEX) team is excited about the opportunity to work with the City of South Bend to help deliver this important Safe Route
to School construction project and hope that we can be considered to serve as your consultant on the construction inspection service for this City of South Bend project.
The City of South Bend plans to construct sidewalks through the safe route to school program to serve the children or students who walk or bicycle to Holy Cross
Elementary School and Muessel Elementary Schools. The project is located on the existing perimeter roads surrounding and adjacent to the school, which consist of
Wilber Street on the west, Vassar Avenue on the north, Allen Street on the east and Lincoln Way West on the south.
Project Team APEX will be a prime consultant, and responsible for all construction inspection activities for this project. The inspection team will be led by project
Supervisor/Project engineer, Mr. Cody Hepler, with support from two consulting firms, who will be assisting APEX with their construction inspection services. These
two partnering firms include Etica Group, Inc. and Abonmarche Consultants, Inc. Etica Group, Inc. will serve as our DBE consulting firm for the project. APEX and
Abonmarche will perform a majority of the construction inspection for the project. Our team has over 45 years combined construction inspection experience.
The Right Qualifications. Your project is our top priority. The APEX Team is committed to this project and has the necessary qualifications and experienced staff to
provide you with an exceptional quality construction inspection service. Our project engineer/supervisor, Cody Hepler is highly knowledgeable and has extensive
experience in overseeing the construction of bridges, roads, trails, sidewalks, and ADA curb ramp projects. Both of our subconsultants, Abonmarche and Etica Group,
have great experience and a reputation in construction inspection on similar projects.
Project Familiarity. The project is located in the west side of the City of South Bend. Mr. Hepler has visited and reviewed the project site to become familiar with the
project. Abonmarche is a local consulting firm who is familiar with the project and will be a great benefit to the project.
We believe our knowledge of this project, along with the past experience of Mr. Cody Hepler and his team members will enable the APEX Team to complete the

Apex Consulting and Surveying (APEX) is a small/medium sized multi-disciplined civil engineering firm located in Fort Wayne, Indiana. The firm is
certified by the State of Indiana as a Minority Business Enterprise (MBE). Since 2004, the company has expanded beyond primarily being known as a land
surveying firm and has grown to include civil engineering services. APEX is focused on providing our clients with professional engineering services of the
highest quality and value. APEX is currently acting as a City Engineer for the City of Garrett, IN.
APEX has made significant contributions to the civil engineering field in the local agencies, and communities in northeast Indiana. We have extensive
experience of working with public and private entities. This team experience will allow APEX to be the choice for this project.

PROJECT UNDERSTANDING
The APEX Team understands that the City of South Bend plans to make improvements to the existing sidewalks, that the children and students
currently walk or ride a bicycle to Holy Cross Elementary School and Muessel Elementary School by replacing the deteriorated existing
sidewalks, constructing new ADA curb ramps to comply with the current standard s, and the need for construction inspection services for this
sidewalks and curb ramp rehabilitation. The project is funded by 90% FHWA and 10% by the City of South Bend. Construction inspection
activities will follow INDOT instructions and guidelines as well as the City of South bend standards. This project will provide for safe walkways
and the aesthetics of the areas. The project is anticipated to be under construction in summer 2024. The project must follow INDOT construction standards
and guidelines, the APEX Team understands that both construction activities and documentation (final construction records) are very important for the project,
however final construction records are the key for deliverability of the project closeout.

APEX TEAM KEY STAFF
CODY HEPLER – Construction Inspection Manger with APEX
Cody Hepler will be Project Engineer and Project Supervisor for this SRTS Holy Cross
Elementary School and Muessel Elementary School project. Mr. Hepler has over 10
years of construction inspection experience working in the transportation industry. Mr.
Hepler has been working on construction inspection services for several types of
transportation projects since 2014. His construction inspection responsibilities include
monitoring construction activities, measuring quantities, collecting field data,
performing various on-site testing, justifying payment for Contractor, preparing
reports, coordination with clients, Indiana Department of Transportation and local
public agency (INDOT & LPA), contractors and subcontractors during construction, as
well as maintaining compliance of specifications of all materials delivered to the project site. Cody is a trusted
partner in ensuring project goals and objectives are met. His past project experience includes bridge
rehabilitation, roadway improvements, pedestrian facilities (trail, sidewalk, and ADA curb ramps), and utility
infrastructure improvements. In addition, Mr. Hepler has been involved with bridge, road, and trail designs for
both INDOT and LPA projects.

• Contract No. R-35906, Waterloo Safe Route to School (SRTS) Phase II, Waterloo, IN. The project consists of new sidewalks and ADA curb ramps,
and reconstructing of the existing sidewalks.
• Contract No. R-34514, Landin Road improvements from Parent Road to North River Road, New Haven, Allen Co., IN. The project consists of road
widening and resurfacing, sidewalks, trail, ADA Curb ramps, and storm drainage.

As part of the City Engineer Responsibility for the City of Garrett, Mr. Hepler has performed construction inspection for the city local projects as well.
SCOTT CARTER - (Etica)
Scott has over 25 years of experience overseeing construction sites, with over 23 years at
INDOT, He is a Site Manager software proficient, knows INDOT standards and
specifications, and is materials proficient. He has experience from start to finish coordinating
with on-site contractors, project management, keeping construction records up to date, and
submitting FCR’s.
His project experience includes the following:

JAMES MCPHAIL - (Abonmarche)
James has over ten years of experience in field operations and construction oversight.
His responsibilities at Abonmarche include construction inspections, progress meetings,
pay estimates, schedule oversight, record keeping, stakeholder communication,
construction staking, utility as-builts, and construction administration.
His project experience includes the following:
• Wild Cherry Ridge Roadway Improvements - Mishawaka, IN. James performed the role of primary
construction inspector for this $600,000 project.
• CR 17 Improvements - Elkhart County, IN. James performed the role of primary construction inspector
for this $800,000 project that included new roadway construction , earthwork, and associated
improvements.
• Southwood Sewer Separation - South Bend, IN. James performed the role of primary construction
inspector for this $2.500,000 project that included new storm sewer and drainage improvements, utility
coordination, and new roadway, curb, and sidewalks.
• Mishawaka Traffic Safety Plan & West Vestibule - Mishawaka, IN James performed the role of primary construction inspector for this $3.2 million
project over seeing all aspects of the construction. This project included the construction of multiple new parking lots, drop off area, and a new grand
entrance into the school. Ensuring ADA accessibility and safe traffic routes for all school traffic.
TAYLOR WILLOUGHBY – (APEX)
Taylor is a stormwater inspector with numerous years of combined experience in stormwater inspections for civil development projects.
His professional background includes stormwater inspections, assisting in stormwater plan development for INDOT projects, and
permitting for various environmental permits. He also has experience with AutoCAD and Autodesk Civil 3D software.
Taylors’s project experience includes stormwater management and inspection of numerous INDOT and LPA transportation projects
including bridges, small structures, pipelining, and pedestrian projects. Taylor is knowledgeable in environmental permits and how they
affect the construction projects and ensuring sites are kept within environmental regulations. He has extensive knowledge in IDEM CSGP
(Construction Stormwater General Permits) and preparation of SWQCP (Stormwater Quality Control Plans) for numerous INDOT and
local projects. His primary duties include stormwater site inspections and SWQCP preparation and permits.

PROJECT APPROACH
The project is located on the existing perimeter roads surrounding and adjacent to the school, which consists of
Wilber Street on the west, Vassar Avenue on the north, Allen Street on the east and Lincoln Way West on the south.
(see inset map on right of this page). The APEX Team has completed preliminary investigations for this Safe
Route to School project (Des No. 2100025) in the City of South Bend. We are moving ahead and are making
great strides with our research. At this early stage, the APEX Team has performed the following initial
investigation tasks:
❖ Mr. Hepler has visited, walked through, and photographed the project site to evaluate the existing conditions
of the existing sidewalks as well as communicating with Ms. Gemma Stanton with City of South Bend.
❖ Reviewed and evaluated the available data such maps and proposal.
Upon receiving the notice to proceed (NTP), we will coordinate with the City Officials who are involved with
the project, Contractor and INDOT La Porte District Area Engineer to get a mutual date for pre-construction
conference. APEX will coordinate and notify all parties involved during construction by sending pre-
construction conference notification letters. The APEX Team will organize and manage the preconstruction
conference. Minutes of the meeting will be prepared, and a copy will be provided to all participants. The APEX
inspection team will meet with the design engineer to learn more about the project and any special design
considerations of the project.
Project Management and Communication
Communication is most important during the Construction project. Lines of communication between all parties will
be established during the pre-construction conference. We plan to have a bi-weekly meeting with all parties involved
with the construction project. Cody will keep INDOT and the City of South Bend updated on the project status
during construction by providing bi-weekly progress meetings with Area Engineer, Contractor, subcontractors, and utilities involved.
The inspectors and project supervisor will record and maintain all documentation in accordance with the INDOT General Guidelines to Field Employee
and Site Manager. Material testing will be completed in accordance with INDOT Procedures. Upon completion of the project construction, preparation of
the project final construction records is our priority.
The APEX Team is committed to providing the City of South Bend with superior construction inspection services. Our goal
