AMENDMENT TO OWNER-ENGINEER AGREEMENT Amendment No. ___1__ 1. Background Data: a. Effective Date of Owner-Engineer Agreement: October 25, 2022 b. Owner: City of South Bend Board of Public Works c. Engineer: DLZ Indiana, LLC d. Project: Angela Boulevard Pedestrian Safety Improvments 2. Description of Modifications: a. Engineer performed the modifications to the plans that were Additional Services beyond the original project scope: 1) The plans were modified to include milling the existing asphalt pavement 1 ½ inches throughout the project limits along Angela Boulevard between SR 933 and 500 feet west of Twyckenham Drive and repaved with HMA surface at a depth of 1 ½ inches. 2) Additional curb ramp layouts and detailed spot elevations were developed at the following locations: a) Curb ramp Northeast corner of Angela Blvd. and SR 933. b) 2 Curb ramps at corners of Angela Blvd. and Leeper Ave. c) 2 Curb ramps at corners of Angela Blvd. and St. Joseph St. d) 2 Curb ramps at corners of Angela Blvd. and Foster St. e) 2 Curb ramps at corners of Angela Blvd. and Stanfield St. f) 2 Curb ramps at corners of Angela Blvd. and Pokagon St. g) 2 Curb ramps at corners of Angela Blvd. and St. Louis Blvd. 3) Pickup topography for the 13 additional ramp locations. 4) An additional bid package will be prepared for bidding. Previously released addendums No. 1 through 3 will be incorporated into a revised plans set and updated specifications. Contract schedule will be revised and quantities adjusted for value engineering plan revisions.
