Michigan St. South Bend, IN 46601 Subject: Repair of Edison Filtration Plant Stairs Date: August 29, 2023 Dear Members of the Board, Water Works Treatment Division is requesting approval for the repair of the front entry stairs at our Edison Filtration Plant. The stairs are cracking and crumbling in spots presenting a safety hazard for those requiring access to the facility. After looking into our available options, we have identified Ziolkowski Construction Inc. as the most suitable option. Most notably their work will include treating the_ underlying rebar before patching (addressing the root cause of the problem), sealing the concrete and applying a textured coating. They will also guarantee their work for a total of $6088.00. Three other companies were also contacted. Gibson and Lewis quoted a patch job that did not include painting or texturing, which would be desired for safety reasons, and they would not guarantee their work for $1946.00. Gibson and Lewis also quoted a full replacement of the stairs for $13,720 which they would guarantee. We also attempted to contact Slatile, they were unresponsive. We also contacted CNS Restorations, but they were not taking any more jobs for this year. Ziolkowski may cost more for the patch, but they will cost less than replacing the stairs completely and they will do the job properly and guarantee their work. The Contractor covenants and agrees to make the following improvement, as fully set out in the Request for Quotations, Standard Specifications, Special Provisions, Plans, and Bid Proposals, all of which are set forth as a part of this Contract, for: DESCRIPTION: EDISON FILTRATION PLANT FRONT STAIR REPAIRS COMPLETION DATE: SEE SPECIFICATIONS AMOUNT: $6,088 FUNDING: PR-00027248 The unit prices for this improvement were those prices as received and accepted by the Board on the 29th Day of August, 2023. The Contractor further agrees to notify the assigned Engineer when this improvement is completed. This notification shall be in the form of a Project Completion Affidavit, signed by the Contractor. Upon final acceptance of the improvement by the Engineer, the Contractor's final estimate will be presented to the Board for final Payment with one (1) copy of the Project Completion Affidavit and one (1) copy of a Waiver of Lien. Each of the Parties, by signing below, represents and warrants to the other Party that he/she/it has the authority to bind the named person or entity to this Agreement.
