Attach a listing of the names and addresses of all subcontractors (i.e. persons or firms outside your own firm who have performed part of the work) that you have used on public works projects during the past five (5) years along with a brief description of the work done by each subcontractor. 3. If you intend to sublet any portion of the work, attach the name and address of each subcontractor, equipment to be used by the subcontractor, and whether you will require a bond. However, if you are unable to currently provide a listing, please understand a listing must be provided prior to contract approval. Until the completion of the proposed project, you are under a continuing obligation to immediately notify the City of South Bend in the event that you subsequently determine that you will use a subcontractor on the proposed project. 4. Attach a listing of equipment you have available to use for the proposed project. 5. Have you entered into contracts or received offers for all materials which substantiate the prices used in preparing your proposal? If not, attach an explanation for the rationale used which would corroborate the prices listed. SECTION III CONTRACTOR'S FINANCIAL STATEMENT Attachment of bidder's financial statement is mandatory. Any bid submitted without said financial statement as required by statute shall thereby be rendered invalid. Larson -Danielson anticipates commencing project coordination upon receipt of a Contract, Notice to Proceed, or Letter of intent. We will then begin awarding subcontracts and purchase orders based on sequence of work to be performed. Work will be performed in common sequence of tasks as generally accepted throughout the construction industry and as it pertains to this project. Average crew sizes range from 5-6 skilled tradespeople, although is subject to change based on schedule and the specific task(s) being performed. 2_ Please list the names and addresses of all subcontractors (i.e. persons or firms outside your own firm who have performed part of the work) that you have used on public works projects during the past five (5) years along with a brief description of the work done by each subcontractor. Please see attached list of subcontractors we have worked with on public works projects in the last five (5) years. 3. Larson -Danielson does plan to utilize subcontractors. Subcontractor information will be provided as required in the relevant bidding documents. 4. Larson -Danielson owns a wide variety of heavy equipment and mid to small sized equipment/tools, including but not limited to equipment for earthwork, masonry, concrete, carpentry, access equipment and transportation equipment. In addition, the Company prohibits any employee being at work or working under the influence of alcohol or drugs, irrespective of the degree of physical or mental impairment the employee may be experiencing, This policy includes the intentional misuse of prescribed drugs as well as the illegal use of unprescribed drugs. Any violation of these rules by any employee while on company business or premises will be cause for disciplinary action, ranging from verbal or written reprimand to immediate discharge and referral to law enforcement agencies. The term "company_ premises" includes all property, buildings, structures, jobsites, parking lots and means of transportation owned by or leased to the company or that which is under the control of the company, the responsibility of the company or otherwise being utilized for company business. In an effort to establish a drug free work place for the safety of its employees, Larson Danielson Construction Co. Inc. will adhere to a drug testing program consisting of, but not limited to initial employment screening, scheduled random testing, and post -accident testing. Consequences for a positive test: Any employee who fails a drug test will be disciplined in the following progression: (A) After the first positive test, an employee will be: (i) suspended from work for thirty (30) days; (ii) directed to a program of treatment of rehabilitation; and (iii) subject to unannounced drug testing for one (1) year, beginning the day the employee returns to work. (B) After a second positive test, an employee will be: (i) suspended from work for ninety (90) days; (ii) directed to a program of treatment or rehabilitation; and (iii) subject to unannounced drug testing for one (1) year, beginning the day the employee returns to work.
