In accordance with all the terms of Chapter 17 of the City of South Bend’s Municipal Code, and with any applicable
provisions of federal or state law and regulations, permission is hereby granted to:

for the contribution of wastewater into the City of South Bend Publicly Owned Treatment Works.
This permit is granted in accordance with the application filed on 29 November 2022.
Effective on: 14 April, 2023
Expires on the: 13 April, 2028
Transferred on the: May 1, 2023
Any violation of the terms and conditions of this permit shall be deemed a violation of the City of South Bend Municipal
Code and will subject the permittee to the sanctions set out in section 17-11 through 17-13 of the Municipal Code.
The permittee shall file an application for a renewal permit, a minimum of 90 days prior to the expiration date.

Dear Board Members:
The Division of Environmental Services recommends approving the transfer of ownership of the
Industrial Discharge Permit for Monarch Textile Rental Services Inc at the April 25, 2023, Board
of Public Works Meeting.
Monarch Textile Rental Services Inc an industrial laundry is being purchased by Randy John,
owner of 401 Linen Services. The new owner has no immediate intent to change the facility’s
operations and processes. The new owner acknowledges full responsibility for complying with
the existing individual wastewater discharge permit. The new owner will be doing business as
Monarch Linen.

PERMIT BRIEFING MEMO
1.Facility Description
Monarch Linen at 1601 Lincolnway West (Monarch) is an industrial laundry. Monarch rents out and
launders textiles including table linen, medical textiles, towels, and aprons to approximately 275 Indiana
and Michigan customers. Monarch operates Monday through Friday from 5:00 AM. to 1:00 PM, up to
3:00 PM. and employs 30 people. Production is subject to seasonal variation.
2. Discharge description:
Discharge is wastewater from industrial laundry washers as well as boiler/tower blowdown. Water is
discharged from the industrial washers into a settling pit. This pit serves as solids removal for the wash
waters. The solids that are floatable are skimmed from the top of the water. The water then is discharged
into the City of South Bend Sewer System through a control manhole located in the south parking lot
adjacent to Lincolnway West.
Monarch has reported that copper and zinc are known to be present in their wastewater at low amounts.
January 2022 – September 2022 the following flow has been observed
Average Daily Flow: 31,836 gallons
Maximum Daily Flow: 60,387gallons
3. Sample Site Description
Sample Site Outfall # 156 is a manhole installed in the parking lot on the south side of the building. The
autosampler hangs inside the manhole during composite sample collection. Grab samples are collected at
the same location.
4. Wastewater Treatment
Wastewater treatment consists of grit removal and oil absorbent socks. The wastewater holding pit is
pumped out periodically by a third party, and then Monarch power washes the pit.
5. Classification
Monarch Textile Rental Service Sample Site Outfall #156 is classified as a Significant Industrial User
(SIU). Monarch Textile Rental Service is categorized as a C industry per the City’s Industrial
Pretreatment Program, which states that all industries discharging process wastewater between 25,000 gpd
and 50,000 gpd are in Category C.
6. Calculation of Limits
Discharge limits are applied from the South Bend Municipal Code without modification.
7. Monitoring Frequency
Monitoring frequencies are based on South Bend’s US EPA approved industrial pretreatment program
monitoring plan.
8. Industrial Operator Certification Requirement
Operator Certification Requirement: A-SO
Industrial operator certification requirements are based on the criteria listed in IAC 5-22-5.
