Dear Ms. Floyd:
At its March 28, 2023 meeting, the Board of Public Works approved the application for
the temporary placement of a sign in the name of John Floyd Dr. at the intersection of Angela
Blvd. and St. Louis Blvd.
Since we received your application prior to 2023 the cost of the sign is $100. We will
refund your $50. Please come to our office on the 13th. Floor to pick it up. Per the Board of Public
Works new policy on honorary street signs (Resolution No. 58-2022), the approved sign may
remain in place for a period not to exceed Five (5) years.
Please contact Denise Miller, Customer Service Coordinator, Street Department, at (574)
235-5939 to coordinate the time and placement of the sign(s).

Melinda Floyd came to the Public Works, Engineering Office sometime between
Wednesday, November 23 and Friday, December 09, 2022 to submit an Honorary Street
Sign application. I can verify this because our front desk Administrative Assistant was on
vacation at the time. When Ms. Floyd came to our office, her application was incomplete.
She had all of the petition signatures and the application, but was missing the written
statement and the $100 application fee, so we told her to bring it back when complete.
She returned to our office on January 25, 2023 with the written statement and the
application fee was paid on February 6, 2023, based on the old resolution.
As Ms. Floyd originally contacted the City's Public Works department in 2022 and
worked in good faith to abide by the then existing rules for an honorary street sign, it is
recommended that the Board of Public Works apply the 2022 standard set forth in
Resolution 48-2009 to this application, including the old fee rate.

To Whom It May Concern:
In recognition of countless sacrifices during his life's work, The Floyd Family humbly requests
commemoration of John Floyd in the town in which he loved and served faithfully for 38 years. Our
father John Floyd worked full-time in The Homicide & Narcotics Division for The South Bend, IN.
Police Department for 38 years. During that time he also worked full-time as a Security Officer for South
Bend Schools at Riley High School. While raising four children (Sonya Floyd, Melinda G. Floyd, Glenis
Townsend Floyd & Christopher Shawn Floyd) he also built an impeccable reputation as a great friend &
distinguished member of the South Bend, IN. Police Department. Our beloved father John Floyd was an
exceptional man that was very dedicated to making a difference in his community; and in doing so he
touched the lives of countless people throughout his life and career.
Education, humility and taking responsibility was very important to our father. He'd often say "Right
is Right & Wrong is Wrong!" Our father was reliable and driven. As such, he encouraged countless
people throughout his life and career. Being a resident of South Bend, I still encounter dozens of citizens
that profess just how our father John Floyd positively impacted their lives. It's an amazing thing to see.
Whether he encouraged them to pursue their future in Law Enforcement or live up to their full potential
in another field ... he empowered them with true love, support and kind words. Many people that knew
our father are just overcome with tears of joy while reflecting on the positive impact that he had on their
lives — how he encouraged them to stay in school, to stay out of trouble ... to better themselves and so on.
Our father was an exceptional yet modest man. We never knew that our father kept a spare change
pouch with him every day while working for Riley High School. While working and communicating
with the students he protected daily, he noticed that sometimes the students would be short on lunch
money; or just didn't have money for lunch. Therefore, he would routinely give students a helping hand
to see them through those tough times. He never mentioned it to us. This was brought to our attention
decades later as former students would recall how "Mr. Floyd" would give them money for lunch in their
times of need. Our father carried this on for many years without mentioning it because to him it was just
the right thing to do.
Ultimately, in understanding and witnessing our father's dedication to the people and city of South Bend,
IN ... we believe that he just wanted to leave this world better than he found it. We truly miss our father,
but we also find comfort and encouragement in knowing his life's work has and continues to impact
many residents of South Bend. IN. To that matter, the legacy of our father's commitment is cemented
in the difference he made with his colleagues in Law Enforcement & the residents of South Bend, IN.

Assemblyman Chad Mayes' proposal needs to fulfill criteria before the names
of Jose "Gil" Vega and Lesley Zerebny may adorn the four --mile stretch of
Highway 111 between overture and West San Rafael drives. The proposal
needs to be approved by the state Assembly and Senate.
Caltrans provided The Desert Sun with a list of criteria.
Here's the criteria:
• Individuals must have provided "extraordinary public service or some
exemplary contribution" to the public and be associated with the
community where the highway is located.
• The designee must be deceased or an elected official who is no longer in
office.
2 Local or private sources need to cover the costs of signs or plaques.
The proposal needs to be done by a person who represents the district
where the road is located and a specific stretch needs to be identified.
• The stretch cannot exceed five mil in -Length,
• A proposal ne,,es to, reflect a community consensus and have no local

1. Application is submitted by petitioner to APC staff, electronically. Please fill out the
application completely, including suffix and public/private section. Incomplete
applications will not be considered and will be returned to the submitter.
2. Staff checks current computer databases for duplicates, previously rejected names, names
reserved for subdivisions not yet completed and phonemes.
3. Staff checks with the Post Office, Emergency 911 and APC Executive Director.
4. Disapproval by any of the above results in a denial for the proposed street name.
S. Changed or Renamed streets are sent on to the jurisdiction's public meeting for adoption.
(If the original name was included in a recorded plat, then the new name will be recorded
by APC).
6. Approved new names are sent to agencies and developers and added to all lists and
databases.
7. Approved new names should be put in the Preliminary Plat or Preliminary PD. Once
recorded all approved names actually used must again be turned into APC staff so the data
base can be updat

SOUTH BEND — John Floyd, the retired South Bend police officer who was severely
injured in a massive house explosion, died late Friday. He was 66.
Floyd died at Bronson Methodist Hospital's burn center in Kalamazoo, where family
members had gathered to be with him, a hospital spokeswoman said this morning.
He had suffered multiple fractures, including two spinal fractures, in the explosion, which

occurred Thursday night at his home in the Arbor Pointe subdivision north of South X
Bend.
Family members said Friday that Floyd had been burned on more than 80 percent of his
body, and that doctors initially told them it would be a miracle if he lived.
Floyd worked as an officer for the South Bend Police Department for 35 years, retiring in
2006.
At the time of his death, he had been working as a security guard at Washington High
School, and had been with the South Bend Community School Corp. for 38 years.
He is survived by three children, 10 grandchildren and six great-grandchildren, family
members said.
Floyd was the only person in his home, which is just south of the Indiana -Michigan state
line and west of Portage Road, when it exploded shortly after 8 p.m. Thursday.
The explosion hurled debris for more than 100 yards, heavily damaged eight nearby
homes, and was heard as far away as Cassopolis and Edwardsburg. Investigators are
working to determine the cause.
