If your event will not be using a pre -approved race route, please explain: There was no pre -approved race route on the Special Events page that I can find. I have attached a map of parade route. This parade has been on this route for at least 40 years (that I can remember). Site Plan / Route Map - For All Events: Provide an attached map with the geographic locations of all event items listed below. • Outline of entire event venue including the names of all affected streets and areas. • Clear markings for street closures and a schedule for each. • Location of fencing, barriers, and/or barricades. Section I - Mitigation of Impact If you are using and/or closing public sidewalks or streets, you are required to notify area business owners and residents. You must: o Present your event concept to the surrounding stakeholders (residents, businesses, and neighborhood groups) that represent the venue area. Attach a copy of the brochure or door hanger distributed to all affected residents/businesses/neighborhood groups describing the event purpose, date and time. Section J - Insurance A Certificate of Insurance (copy) confirming the existence of a liability policy (General Liability and Automobile Liability) of not less than $700,000 per occurrence and $1,000,000 aggregate, which specifically names the City of South Bend, IN as an additionally insured for the event must be submitted. Pursuant to Local Ordinance No. 10628-18, there is a $50.00 non-refundable fee for Tier II and III event applications filed 60 or greater days in advance of the event, or a $100 non-refundable expedited fee for applications filed between 30 and 59 days in advance of the event. 2. The APPLICANT must comply with all terms and conditions of this Permit and Agreement. 3. The APPLICANT must obtain signatures from and/or make an attempt to notify all residents that reside on the block. A copy of a brochure or door hanger distributed to all affected residents/businesses describing the event purpose, date, time and contact information must be included with the attachments to this application. The applicant is responsible for providing affected residents and business owners with transportation to their property. 4. The APPLICANT shall reimburse the Board for the actual cost to the City for the event, if deemed necessary. 5. Notification of approval/denial of this request will be issued by return of this form, upon signed authorization by the Special Events Committee. 6. The APPLICANT shall provide to the Board a Certificate of Insurance showing a liability policy in full force and effect with limits of $700,000.00 per occurrence and $1,000,000.00 aggregate and the City of South Bend, Special Events Committee, and Board of Public Works listed as an additional named insured for this event. 7. The APPLICANT assumes full responsibility for providing ample disposal containers for refuse/recycling and assures the area will be cleaned up upon the conclusion of the event. 8. Barricades will be delivered and picked up at the event location. The APPLICANT is responsible for seeing that all cones are maintained and returned undamaged. 9. The APPLICANT will follow the City of South Bend Noise Ordinance, which is in effect at all hours. Between the hours of 11:00 p.m. and 7:00 a.m. certain noises are particularly prohibited. These include operating radio receiving sets, musical instruments, phonographs and other sound reproduction devices if audible fifty (50) feet away, as well as shouting, yelling, hooting, whistling, or singing in the streets in a manner to disturb the peace (Municipal Code 13-57). 10. All events should meet this requirement regardless of crowd size. First Aid -First Aid kit will be at beginning and end of parade Event Headquarters/ Information Center All staging volunteers will have safety vests and radios. Weather • Thunder & Lighting: Event will be cancelled • Wind: If wind gusts are 35 mph or more, the staff will decide if it poses a hazard to the attendees. Marshall will have the ability to cancel the event for public safety justification. • Tornado: In event of tornado watch or warning, event will be cancelled Lost Child • Go to Volunteer w/vellow vest and radio and notify parade board member to announce over the radio system "_ age child looking for _ (parent, aunt, grandma). Meet child at the _ -Stay with child until reunited with parent. • If a parent comes to you saying they lost their child: - Go to volunteer w/vellow vest and radio and notify board members to announce over the radio system the child's first name and description of clothing. - Available staff needs to search the immediate area, restrooms. - If child is still not found, call 911. ACTIVE SHOOTER An Active Shooter is an individual actively engaged in killing or attempting to kill people in a confined and populated area; in most cases, active shooters use firearms(s) and there is no pattern or method to their selection of victims. Active shooter situations are unpredictable and evolve quickly. Typically, the immediate deployment of law enforcement is required to stop the shooting and mitigate harm to victims. Because active shooter situations are often over within 30 to 15 minutes, before law enforcement arrives on the scene, individuals must be prepared both mentally and physically to deal with an active shooter situation. Good practices for coping with an active shooter situation: o Be aware of your environment and any possible dangers o Take note of the nearest exit routes away from the shooter. o Help other escape with you. o As a last resort, attempt to take the active shooter down. When the shooter is at close range and you cannot flee, your chance of survival is much greater if you try to incapacitate him/her. CALL 911 WHEN IT IS SAFE TO DO SO! How to Respond when and active shooter is in your vicinity • Quickly determine the most reasonable way to protect your own life. Remember that event participants are likely to follow the lead of our staff during an active shooter situation. • Evacuate. If there is an accessible escape path, attempt to evacuate the premises. Be sure to: • Evacuate regardless of whether others agree to follow • Leave your belongings behind • Help others escape, if possible • If you are out in the park evacuation is not a safe option, attempt to take shelter behind a building or lie flat on the ground if shots are being fired through the air • Prevent individuals from entering an area where the active shooter may be • Keep your hands visible • Follow the instructions of any police officers • Do not attempt to move wounded people • Call 911 when you are safe Hide out. If evacuation is not possible, find a place to hide where the active shooter is less likely to find you. Your hiding place should: • Be out of the active shooter's view • Provide protection if shots are fired in your direction (i.e., a Tree or wall) • Do not trap yourself or restrict your options for movement • Silence your cell phone and/or pager • Hide behind large items • Remain quiet If evacuation and hiding out are not possible (a very real possibility in an open area) • Remain calm • Dial 911, if possible, to alert police to the active shooter's location • If you cannot speak, leave the line open and allow the dispatcher to listen Take action against the active shooter. As a last resort, and only when your life is in imminent danger, attempt to disrupt and/or incapacitate the active shooter by: • Acting as aggressively as possible against him/her • Throwing items and improvising weapons • Yelling • Committing to your actions How to respond when law enforcement arrives Law enforcement's purpose is to stop the active shooter as soon as possible. Officers will proceed directly to the area in which the last shots were heard. • Officers may wear regular patrol uniforms or external bulletproof vests, Kevlar helmets, and other tactical equipment • Officers may be armed with rifles, shotguns, handguns • Officers may use pepper spray or tear gas to control the situation • Officers may shout commands, and may push individuals to the ground for their safety • Remain calm and follow officers' instructions • Put down any items in your hands (i.e., bags, jackets) • Immediately raise hands and spread fingers • Always keep hands visible • Avoid making quick movements toward officers such as holding on to them for safety • Avoid pointing, screaming and/or yelling • Do not stop to ask officers for help or direction when evacuating, just proceed in the direction from which officers are entering the premises Information to provide to law enforcement or 911 operators: • Location of the active shooter • Number of shooters, if more than one • Physical description of shooter or shooter/s • Number and type of weapons held by the shooter/s • Number of potential victims at the location Notes: The first officers to arrive to the scene will not stop to help injured persons. Expect rescue teams comprised of additional officers and emergency medical personnel to follow the initial officers. These rescue teams will treat and remove any injured persons. They may also call upon able-bodied individuals to assist in removing the wounded from the premises. Once you have reached a safe location or an assembly point, you will likely be held in that area by law enforcement until the situation is under control, and all witnesses have been identified and questioned. Do not leave until law enforcement authorities have instructed you to do so. Bomb Threat Bomb threats are usually reported via telephone. If you receive a bomb threat via telephone: • Take the caller seriously. • Ask a lot of questions, (see list below). • Take notes on everything said and heard, including background noise, voice characteristics, etc. • Keep the caller on the line as long as possible by asking questions (If the caller hangs up do not use the telephone on which the threat was received.) • Call police immediately after call from another telephone or ask another person to call the police immediately. • Do not search for the explosive device or touch any unusual or suspicious objects. Questions to Ask the Caller • When will the bomb explode? • Where is it? • What does it look like? • What kind of bomb is it? • What will cause it to explode? • Why was it placed in the building? • Did you place the bomb? • What is your name? • Observe the Caller Try to identify the following about the caller: • Caller's gender • Approximate age • Voice characteristics, accents, etc. Is the voice familiar? • Background noises Suspicious Package Found If a suspicious package is found, do not touch it, tamper with it, or move it. Call 9-1-1 and let them know a suspicious package has been found. Include package characteristics in your emergency call. Suspicious Package Characteristics: • Appearance — Stiffness or springiness of contents; protruding wires or components; oily outer wrappings or envelope; feels like it contains a powdery substance. • Smell — Particularly almond or other suspicious odors. • Writing — Handwriting indicates a foreign style not normally received, cut -and -paste letters or rub on block letters are used. Common words, titles or names are misspelled. Security • Police will be in attendance on and off during the event. • South Bend Police has been advised of the event. Toilet Provision • 2 port -potties placed on the parade route (refer to attached site plan) — including one disability toilet. Hand sanitizer is available in the portable restrooms. Volunteers • For the purpose of this document the term "Volunteer" refers to all unpaid personal volunteers are being used to assist with parade. • Volunteers are to be prepared for all weather conditions and will be provided with this health and safety plan and other relevant information for this event. General Guidelines All event volunteers to: • Report hazards or incidents • Observe and obey all warning signs/instructions • Report all injuries • Get first aid promptly • Report all fires and emergency situations • Only use equipment you are certified trained or have the skill to use Crowd Control Hazards • Inappropriate event participant and/or spectator behavior (Police) • Excessive consumption of Alcohol or illegal substances • Crossing Roads • Volunteer Staff to monitor crowd behavior and advise Board Members. • Inform Board Members of inappropriate behavior or if a situation is imminent. • Advise Event Coordinator if Police intervention is required. • Contact Police if situation looks to escalate to violence. • Where illegal substances are in use then the Police shall be contacted in all circumstances without exception. This parade is nearly 100 years old and is in memory of our veterans who have passed. The parade will run from 8:15am to loam.
