Included are the following documents for closeout:  Project Completion Affidavit  Maintenance Bond  Final Waiver of Lien  Final Pay Application No. 3 – Release of Retainage Summary of Project – Final Costs: Original Contract Price $124,980.00 Change Orders (None) $ 0.00 Change Orders, Percentage Change 0.00% Final Contract Price: $124,980.00 On Budget This project was completed on budget. There was a change in the MWBE Utilization Plan that occurred during construction. City Engineering Team directed Selge Construction to remove the old concrete lid from CSO 18 for use at the CSO 27/28 collapse and repair project to save on costs for the CSO 27/28 manhole structure repairs. This allowed for re-use of the material for the repair project. Consequently, V&R Trucking had originally been scheduled to haul this item away from CSO 18 for demolition, but the lid was hauled by Selge Construction to the CSO 27/28 site due to scheduling needs for the CSO 27/28 project. This resulted in a minor change to the MBE Utilization Plan. This change is being recognized with the Final Completion documents. Refer to attached memorandum from Selge Construction Company explaining the adjustment to the MBE Utilization Plan. Closeout documents are submitted for approval at the 12/05/2022 Board of Public Works Meeting. For questions regarding the subject agenda request, contact Jacob M. The total amount of this commitment was $210.00 and accounted for .16% of the total Bid/Proposal. The work that V&R was to perform was hauling the old concrete lid to a dump site to dispose of the lid. During the work of this contract an emergency repair for the City of South Bend occurred. Selge Construction Company was directed to take the lid to be used on this emergency repair. Therefore, we were unable to contract with V&R trucking to perform the work we had selected them to perform. Thank you for your understanding in this matter.
